My cat has kidney failure and anemia, what should I feed her?

Hi there. Unfortunately, it's not likely that liver or any other food will make much difference in his red blood cell count. Anemia in chronic kidney disease is due to the body not producing erythropoietin, which is the hormone that drives the production of new red blood cells. This type of anemia can be treated with erythropoeitin injections, so I would recommend talking to your veterinarian about this option. I have had several kitties with chronic kidney problems and anemia who responded well to this treatment.

If he won't eat the Royal Canin renal diet then you should try a different brand like Hill's k/d. The Hill's diet has an appetite stimulant in it which helps with palatability. If he won't eat that either then all you can do is feed him what he will eat. It may shorten his life span but you cannot let him starve to death. If he is anemic that is called anemia of chronic disease. The only thing to help that is injections of EPO (erythropoeitin). Feeding organ meet is not going to help. Find a food with a strong smell/odor. Warm it slightly then offer it to Precious. You can also mix a teaspoon of warmed baby food in with her food. Discuss appetite stimulants, such as mirtazipine, with your veterinarian. The vet may also suggest some kidney friendly diets such as Hill's k/d or Royal Canin Renal Support. Bloodwork should be submitted to check kidney function. Elevated BUN and creatinine levels may cause nausea and inappetance. Hospitalizaton and IV fluids may be necessary to decrease levels.

Prescription kidney diets have proven to improve both quality and length of life in pets with poor kidney function. They are formulated to reduce the amount of work required for the kidneys by reducing the amount of waste products produced from food as well as containing tightly balanced mineral, vitamin, omega fatty acid and protein levels and work best in wet forms or where good water intake can be encouraged. The larger proportion of the diet the better, any additional food sources should be highly digestible and suitable for pets in kidney failure, blood tests would be recommended 3-4 weeks after any diet changes to ensure no negative impacts develop

These lab results do not provide any "normal" values, which can differ based on the type of equipment used to run the sample, however I would agree that based on what I am used to as normal values Sabrina is in either renal failure or has very severe dehydration. Renal failure seems more likely given that she is also quite anemic (has low red blood cells), although her liver values do not look high to me. Typically renal failure is treated with aggressive intravenous fluid therapy, and with kidney values this high it seems unlikely that only getting fluids every 12 hours will make much of a difference. Also, with anemia such as she has, great care has to be taken not to give fluids too rapidly as to worsen the anemia. Persian cats are predisposed to a type of kidney disease known as polycystic kidney disease. This is easily diagnosed with ultrasound examination of the kidneys. Unfortunately, the prognosis is grim.

These two diseases are very frustrating to manage as the kidneys need fluid to help "flush" them but the heart cannot handle any additional fluid. Additionally many drugs used to treat CHF cause harm to the kidneys. With anemia and kidney disease there are synthetic hormone injections that stimulate red blood cell production. Your veterinarian has to administer these injections. Sometimes even a blood transfusion is given but this has to be balanced with the heart failure. I recommend discussing with your veterinarian which disease process is most critical right now and start treating that as aggressively as possible. This would likely mean hospitalization for Anthony.
