My cat has kidney disease and anemia. Should I try EPO shots?

Hi there. So sorry to hear that Pisces is having this issue. EPO can stimulate red blood cell production and in theory it sounds like a good idea however, it has it's own side effects and some are very serious. Also EPO is outrageously expensive so it is not a very cost effective treatment. Most cats will do well with blood transfusions and treating the underlying cause of the anemia (if one can be found). Hi there. Unfortunately, it's not likely that liver or any other food will make much difference in his red blood cell count. Anemia in chronic kidney disease is due to the body not producing erythropoietin, which is the hormone that drives the production of new red blood cells. This type of anemia can be treated with erythropoeitin injections, so I would recommend talking to your veterinarian about this option. I have had several kitties with chronic kidney problems and anemia who responded well to this treatment.

The best diet for a cat with chronic renal failure is a canned-only, low-protein diet - this won't help with the anemia, specifically, but is the best option to help slow the progression of her kidney disease in general. You can try supplementing her with an iron supplement designed for cats such as Pet-Tinic - this can be ordered online without a prescription, and has dosing instructions on the label based on your kitty's weight.

Thank you for submitting your question regarding Mellow. This would only help if the anemia is due to an iron deficiency. This is not that common though. Most cases are due to an immune mediated disease. Kittens though can be affected with parasites leading to anemia. It is best to work with his veterinarian to determine the source of the anemia so that most appropriate treatment can be started. I hope this information helps!

Ouch poor Harry. Low PCV could be from a number of things including immune-mediated anemias, lack of blood cell production or as you checked, problems with the kidneys and erythropoetin production. If the kidneys seem functional looking toward issues that would cause auto-cell destruction may be the next step as well as possible transfusion or injection of EPO to stimulate red cell production. Checking for parasitic anemia and high parasite load may also give clues to the low PCV value. Panleukopenia, FIV and FeLV can also cause low PCV as part of the disease process. Ruling out an internal bleed somewhere may also help narrow down the cause.

The pale gums do signify anemia that could be related to kidney disease, cancer, bone marrow disease and other significant issues as well. I think your ideas are good and could be beneficial although they do not address the underlying problem. However good nutrition is always a good thing. Cats that are hard to handle are challenging and diagnostics are the only way to know what is the cause of the anemia. Certainly it is concerning that she did not do well with sedation. You may contact her vet and ask about an option of an oral light sedative called Gabapentin that tends to work well for aggressive cats without the side effects of stronger sedatives. It doesn’t work for every cat but I find it to be effective more often than not. Your vet will decide if this is an option based upon your pet’s overall health and previous medical history. Poor guy. Non-regenerative anemia in an older cat is always concerning. Due to the timing of all of these events however, his anemia could have very well have been caused by the medication alone. This would be considered to be an adverse reaction that would not be normal for any pet to have not can be predicted. But with that said it usually is a regenerative anemia when medication is the cause. A closer look at his bloodwork could help clarify this. Other possible causes that are common in older cats are: feline leukemia/feline aids or cancer unfortunately. Typically with all of these you will also get some changes with the white blood cell count levels as well and often-times a fever. Overall the prognosis would be guarded to poor considering your pets age and the fact that the anemia is non- regenerative. I am so sorry to be bearer of bad news here.
