My white cat's ear is red, warm, and twitching. Sunburn or infection?

This is a serious problem as cats with white ears and lots of sunburns are very prone to cancer on the ears (Squamous Cell Carcinoma). The best would be to make your cat an inside only cat in summer so that she is not exposed to the sun. This includes pulling shades in the house so she cannot sunbathe and get burnt in the window either. You can try to put sun lotion on the ears with a high protection factor against UV light, however most cats will lick it off. Keeping her out of the sun is vital and the only way to keep her from being burned.

These are probably not sun burn areas, as those would be red and ulcerated in appearance. Instead these are probably freckles. It's probably an ear infection, often it cannot be seen from the outside. Ear infections are very common in cats, it is usually caused by either yeast or bacterial disease. the treatment is usually simple and straightforward, you will have to take him to the vet and get the ears checked, then a prescription ear drops will be prescribed according to the infectious agent.

Sammy could have ear infections, insect bites, sunburn, or even early cancer. Without knowing what the causes is it is difficult to say what treatment you could try. If it's due to insect bites then keeping him indoors and away from any insects is appropriate. However if he has an infection or this is early cancer your veterinarian will need to be involved and likely prescribe medication or even need surgery.

The two most common causes for lesions/irritation in this region would be an ear issue (the cat is traumatizing the skin while trying to get to the ear) such as infection, mites, or foreign body within the ear; or allergies (we can have allergies to foods or environmental allergies like pollen, dust).
