Why does my cat have chronic explosive diarrhea despite treatments?

HI there. So sorry Onyx is having this issue. If those medications are not working I would be worried about the possibility of something else going at. She may not have IBS at all. My suggestion would be to get some intestinal biopsies so you can have a definitive diagnosis. Without a diagnosis all your vet is doing is guessing as to what may be wrong. Chronic diarrhea in an older cat with vomiting can be a sign of an inflammatory bowel disease or intestinal cancer. The only way to differentiate between the two disease is a surgery to take biopsies of the intestines and have them analyzed. Based on the diagnosis, the diet may be changed or vitamin supplements or steroids may be added to manage her symptoms. They will check for parasite and check her labs for any renal or endocrine disease

Some possible causes of persistent diarrhea include parasites, inflammatory bowel disease, gastrointestinal cancers, liver disease, food allergies, among others. Mix his old and new foods together over 5-7 days, then exclusively feed a new diet for at least 8-12 weeks to determine if the change has been effective. If these cats have not been on long-term medication like probiotics and metronidazole, I would start these. Also, all cats might benefit from a food-allergy diet trial like Hills z/d fed exclusively for 8-10 weeks. This helps to rule out food allergies as a cause for chronic diarrhea. If these things don't work, you may have to actually send out fecal tests for culture and sensitivity to have done at an outside laboratory as most vet clinics are only equipped to do basic parasite fecal floats and smears in-house. Your female who is having behavior issues may benefit from anti-anxiety medications like Zylkene or Lorazepam, or even an anti-depressant. Since all three are young cats, it's most likely that they are sharing a parasitic or other intestinal infection that is causing chronic diarrhea, so I think you are on the right tract to continue to monitor for coccidiosis and giardiasis. Poor Lily! Diarrhea even in old cats isn't normal. I have diagnosed parasites especially giardia in cats who are strictly indoors. You could try a sensitive stomach food and some probiotics to see if this helps. If not, I would strongly recommend your vet to take a look at her. Your vet will likely do a fecal and possibly blood work, thyroid test and x-rays. I would also make sure they rule out giardia and campylobacter. In older cats, I worry especially worry about inflammatory bowel disease and GI cancer. If your vet is concerned about this, they will be able to talk to you about potential diagnostics (Ultrasound and GI panel, etc) and treatments to help her feel better. From my understanding of your post, you have been giving Proviable to Misty for many months now, and while it seems like it initially worked, she is still having diarrhea. I hope I have that correct! Because she has been having diarrhea for such a long time and the Proviable has been given to her for many months, I would say it's not working for her, and I recommend taking her into the vet again as soon as possible. A year is a very long time for a senior cat to have diarrhea, and she may need further tests to diagnose the cause of her diarrhea such as a geriatric blood panel, urinalysis, and possibly x-rays of her abdomen. The cause of the diarrhea needs to be diagnosed so she can be treated correctly. I recommend bringing her into a vet tomorrow. Good luck!

It's not clear from the history what sort of duration we're talking about, but it can take time (sometimes weeks) to get IBD symptoms under control. Many cats improve much faster if put on a tapering dose of steroids (often prednisolone is used), so you could talk to your vet about this. Depending on what diagnostics were performed that led to the IBD diagnosis, those symptoms could indicate IBD, but could also be consistent with a food sensitivity/allergy, or GI lymphoma, among other things. An abdominal ultrasound helps with a diagnosis, if that hasn't been done, with a GI biopsy being the ideal diagnostic (although that's also quite invasive, and your vet can help you decide if it's worth the risk). At this point, I'd recommend communicating with your vet and letting them know how Port's doing, as they may have been hoping his symptoms would improve with the medications you mentioned - but they may want to change things (such as adding a steroid, or discussing the next diagnostic step) once they know he's still having diarrhea.
