Whether or not it is safe to leave your dog at your sisters will depend on your dog. If he has never been around children before or has shown any aggression towards children, then it is less safe. No dog should be left unattended around children, so it is best if your dog hasn't been around the baby to keep it in an area separated from where the baby will be- either a separate area of the house or outdoors. Some dogs are fine with babies, others may be made anxious by the presence of a baby or they may view the baby as a prey due to small size and the funny noises the baby makes. Since your dog is so small, perhaps putting it in a pen inside your sister's house would work. He shouldn't be hard to contain due to his size. Whether you should bring him with you is going to depend on where you area going- do they allow dogs? will it be very hot outside? will you have to leave the dog in a car? It can get hot VERY quickly in a car in warm weather even with windows rolled down, so it is not recommended to leave a dog in a car in the summer with warm weather, so unless the places you are traveling allow pets it may be best to leave him behind. Keeping him cool will involve providing access to fresh water, shade, and not leaving him in a car or outdoors for extended periods of time in the warm weather. For days where it is very hot, you do want to avoid taking your dog for a walk at the hottest parts of the day (mid-day) especially on paved areas as your dog's feet can get burned. They do sell dog booties that you can put on your dog's feet to help protect them. If she has no skin/coat issues, just a gentle oatmeal shampoo is fine for bathing when needed. Most dogs do not need frequent bathing unless they are getting very dirty and bathing too often can dry their skin out. I wouldn't bathe more than every other week unless needed. If they are not, I would ask your vet what products are available where you live for these issues.

There are cooling mats available that you can take with you to help keep him cool. Make sure there is always fresh clean cool water available for him to drink, even if you have to carry it yourself. Never walk him on hot pavement in the summer. You may consider also taking cold packs with you to supplement the cool pad and/or a damp towel for him to lie on.

I would recommend looking into the requirements for your dog to travel to this island. Most islands are rabies free and you will need a rabies titer which can take 6 plus weeks to get back. this titer has to be sent to a very specific lab and also you will need a international health certificate writen by a usda vet. It might not be worth the stress and expense of travel to take him with you for a vacation

He is most likely getting so got because he is really anxious. Firstly you could use active cooling measures such as letting him sit on a damp towel and damping him slightly with a cloth or sponge. Secondly you could take him to the vet as they may be able to prescribe meds that will help control his anxiety, and thus make him cooler.
