What oral or external medicine is safe for guinea pig wounds?

That looks like a laceration from the photo. I recommend to have Sussy examined by the veterinarian who may want to put her on antibiotics. In the mean time, you can gently wash the area with soap and rinse it very well. Pat dry the fur well. Make sure to keep her warm until she is dry. Disinfection can be possibly started at home with very diluted hibiscrub or betadine or salted water baths (pay extra care when you do it as the eye is very close). Yes you can use Betadine solution (not the scrub). You will want to mix it with tap water until it is a light iced tea color. Don't use it straight. Chlorhexidine solution would be better to use in my opinion. Be aware that this is not an easy thing to cure and it will take a long time. I recommend soaking the feet with with either chlorhexidine or betadine, drying the foot and applying Neosporin twice daily. Continue the soaks until the ulcerations are gone, but continue the ointment until completely healed. I suggest bedding on towels and changing it daily. If he is in a wire cage you need to take him out of it immediately. Hope this helps. Best wishes.

Even though you do not see an obvious puncture wound, i assure you there is at least one (sometimes too small to see), he will need to have an antibiotic course in order to recover from this, he might also need pain killers for a few days. i strongly recommend taking him to the vet ASAP in order to get it treated.

Poor Betty this looks sore! I would say this is due to an infection of some sort. It could be bacterial even coming up from the resp tract but the flouroquinilone should have helped. Otherwise it may be a parasite or fungus such as ringworm. I would get a skin scrape checked and have a hair brushing sent off for ringworm. I know you have tried antifungal cream, sometimes canestan works but at times they need stringer treatment from the vet. You could also try an antiparasite treatment such as xeno.
