Why does my Shih Tzu cough like a furball with an enlarged heart?

Hello His cough previously could've been due to something else and now it's due to his heart disease. Unfortunately without x-rays done at the time of his previous coughing episodes we won't know for sure. However it is very unusual for dogs to go a long period of time in congestive heart failure without treatment and do well. So it's doubtful his heart failure was causing his coughing before now.

Hello and welcome to Petco Pet Education Center, formerly Petcoach. Cyst: The image of the mass looks like it is likely a cyst however you cannot rule out things like cancer without taking a sample. If it is not growing, is freely mobile, and doesn't bother Princess, then you can ignore it. Otherwise, you can have it removed and sent away to be sampled. Cough: Coughing is concerning as an early sign of heart disease, or early sign of collapsing trachea. I would recommend a visit to the vet to check for a murmur, and also possible x-rays to see if her heart is enlarged or her trachea is small. Eyes: Cloudiness can be either nuclear sclerosis (age changes of the connective tissue in the lens of the eye- this is not a concern and does not cause blindness), or cataracts. Cataracts do cause blindness and can be due to age, or other diseases such as diabetes. The darkness over her eye could be due to a disease called 'dry eye' where the eyes don't produce enough tears. Overall, there are some conditions here that can be tested for fairly easily, and treatment is relatively minor (medications, eye drops, etc.). I would recommend having her into the vet for a check up. Best of luck with Princess.

Hello and welcome to Petco Pet Education Center, formerly Petcoach. Coughing and bringing up clear liquid could be coughing up stuff instead of vomiting. This is important to try to differentiate. Coughing up liquid is more consistent with heart disease. The x-rays are difficult to read as the image transfer through is of poor quality and to really assess the lungs you need good quality images, however the heart does look a bit enlarged and the lungs do look a bit 'white'. I cannot assess the liver on these radiographs as you cannot see them. An enlarged heart, white lungs (fluid in the lungs), and an enlarged liver is consistent with both right and left sided heart disease. I recommend you return either to your vet, or another vet, and get a specialist ultrasound of the heart (an 'echo') to determine if heart medications are required. Your vet can also listen to the heart to see if a murmur is present in the heart, or 'crackles' are heard on the lungs. Heart disease is very serious and needs medications to manage. Without these Lily unfortunately will essentially drown in her own lungs, if heart disease is the problem. I suggest a recheck as soon as possible to get this figured out. It could be some tracheal irritation due to dry air or residual congestion from laying down all night. If he isn't coughing all day/night, exhibiting lethargy or does not tire easily then just mention it to your veterinarian during the visit. Your vet will likely listen to the heart and lungs during the exam. Chest xrays may be recommended if indicated by something found on exam. Sparky should have a heartworm test if this is his yearly exam. Heartworm disease can cause also coughing as can various infections.

I agree that Duke heart looks enlarged and it is pushing up on his trachea. I cannot diagnose his condition without doing an exam but with just looking at the X-rays, I would say he definitely has a heart condition. I have heard coughs from heart problems sound very similar to upper respiratory infections. Your vet will be able to put him in some heart medicine to help him. Heart medicine will slow down the progression of heart disease and help him feel better. I hope this helps. I'm sorry for the news!

Based on the radiograph you attached, the vertebral heart score (VHS) is on the larger measurement and overall the heart does have increased contact with the sternum. There does not appear to be evidence of heart failure, but I am limited with one x-ray view and no ability to adjust the contrast. I don't know what medication your veterinarian started Bono on but in this case, I would recommend starting omega 3 fish oil, Pimobendan (prescription medication that helps delay on the onset of heart failure and helps the heart pump more efficiently). I would also look into heart supplements such as Standard Process Whole Body Support and Cardiac support (if available in your country). Here is an article from my hospital on heart disease and what to look out for. Mitral valve disease is one of the most common heart conditions we see in dogs. I am sorry to hear about Abbey's problems! Enalapril is used for heart disease as well as other problems such as kidney protein-loss and high blood pressure. We do, however, like to monitor kidney values via bloodwork when giving enalapril along with lasix. Side effects of enalapril can include nausea, diarrhea, or inappetance. In some dogs we can see listlessness or lethargy as a result of drop in blood pressure. Some dogs can also get elevations in potassium levels. It would be best to have a more thorough workup if your vet suspects fluid in the lungs. A cough can be the result of numerous causes in older dogs and the only way to truly tell if there is fluid buildup in the lungs would be an x-ray. The x-ray would also let us evaluate heart size and check for other causes of coughing (infection, tracheal disorders, chronic bronchitis, masses). Depending on what is seen on the x-ray, an ultrasound of the heart may also be recommended (if we do see enlargement of the heart and fluid build up in the lungs this would be the next diagnostic step). Bloodwork would also be recommended to check the kidney values and the potassium levels when on this medication.
