My senior dog won't eat, keeps falling, and acts strange.

I would definitely recommend some blood work, if this hasn't been done recently. Older dogs are prone to a number of health issues that can cause decreased appetite along with being less active - chronic kidney disease and hypothyroidism are two possibilities that are both very common. In many cases, these issues are treatable (at least to some extent) once a diagnosis is made, so it's well worth checking for them if you can - hopefully, there are some things you can do to make him feel better for as long as possible, even though he's getting older.

Possible reasons are: pain, fever, nausea. These things could be causes by arthritis, lyme disease, kidney, liver or gall bladder problems or even cancer. Sasher does sound pretty sick if she hasn't eaten for three days, and unfortunately anorexia is a general sign of many different illnesses. Things that could be causing this include dental disease, liver failure, kidney failure, heart disease, systemic illness, injury, or cancer to name some. The sooner you get her into the vet, the better. I'd take her now. Best of luck.

But I can outline my thought process for you. The first thing I would want to rule out is any source of pain on her physical exam. Pain from dental disease, arthritis or back pain can all affect the appetite. Then I would do blood work and a urinalysis. The blood work should include a complete blood count, chemistry panel and a test for pancreatitis known as a cPL. Next, I would do chest x-rays and an abdominal ultrasound. I may consider a trial course of pain medication or an anti-nausea medication. Poor Milly! That is very concerning. I would recommend taking her in for an examination by a veterinarian. Falling can be a sign of cardiac disease or neurological disease. Sometimes it can be something like Old Dog Vestibular disease which causes severe neurological signs like circling, nystagmus (eyes bouncing back and forth) head tilt and falling over. This condition usually self resolves, but needs a veterinarian exam to differentiate from a more serious condition. Sometimes it can be a heart problem which is causing poor circulation which would require medications. A veterinarian will do an exam to localize the problem to a system and then blood work .
