My senior dog has painful loose teeth and heart issues. What can I do?

I'm so sorry Peper isn't doing well at the moment! I work for a home euthanasia service, so this is a question I face with my clients regularly. It does sound like he has some dental problems, and you are correct that it is painful when a dog has loose teeth. The good news is that this isn't necessarily an issue that can't be fixed. Peper could have a dental cleaning, scaling, and polishing by the vet who can remove any loose or broken teeth, which would relieve his pain. Having those painful teeth out will greatly improve his quality of life and extend his life. I would have Peper seen by a vet tomorrow to discuss doing a dental procedure on him.

I’m sorry to hear about the tooth loss. It sounds like you are trying hard to maintain good oral health and you should continue this. Definitely try to brush the teeth every day, even twice a day if possible. Pet tooth paste helps but the main benefit is the physical brushing. They also make dental diets which can improve oral health. Some pets also go on pulse therapy antibiotics to help control bacterial populations in the mouth. Unfortunately despite best efforts, dental disease can progress resulting in a he tooth loss. A fair amount of dental disease is found under the gumline and so it is hard to address without a proper dental cleaning. You may ask your veterinarian if there is an option to have you pup cared for by a specialist who may be able to preform a dental cleaning. If this is an option. The recommendation would be to remove the tooth but with the concurrent heart disease it makes the decision difficult to put him under anesthesia. I will say, I have placed numerous pets with heart failure under anesthesia that did just fine. This just required an adjustment of the medications and careful attention to the heart monitor to catch any changes early. I would not suggest pulling the tooth at home. I would recommend continuing antibiotics every other month to prevent infection in the mouth. I might also consider speaking with a dental specialist about removing the tooth. Need Dental Specialists deal with many high-risk cases. I would also make sure that you are addressing the discomfort with some kind of anti-inflammatory. If Chico likes playing with toys, I would provide Chico with hard treats or toys to chew on. These Carter objects may help dislodge the already loose tooth. If your vet is not comfortable with performing anesthesia on your pet, ask for a referral to a veterinary neurologist. Jodie may need to start medication for her seizures if they are becoming more frequent, and the neurologist can help you with that and also help discuss risk vs benefit for a dental. Denta disease and broken teeth are extremely painful, and should be addressed if the neurologist thinks it is safe. A veterinary anesthesiologist can also be recommended to perform the anesthesia during the dental if needed. In the meantime, keep a seizure log to monitor what is happening, and if Jodie has a seizure lasting more than 5 minutes take her to the emergency hospital, as this can be dangerous.

It can be very scary to consider anesthesia in a dog with heart and kidney issues, for sure. But rotten teeth are also a very significant health issue. They become easily infected, and bacteria can get in the blood stream and will stick to areas that aren't functioning normally, like heart valves and kidneys. So by leaving the rotten, loose teeth in the mouth, you are at an increased risk of causing damage to the heart and kidneys. There are many precautions we take when considering anesthesia in these patients, but ultimately we do it to prolong their lives.

It is never safe to do a non-anesthetic dental, and it should never be preformed on a dog. I can understand your concern, but Daisy does need that loose tooth removed and her teeth cleaned.
