Is Rimadyl for a senior dog's cough and suspected enlarged heart?

I am sorry to hear that Mattie isn’t feeling well. Rimadyl is great for arthritis pain and problems that improve with an anti inflammatory medication. However, without know what is wrong with Mattie there is a possibility that the Rimadyl could make things worse. Such as if she is suffering from liver disease, pancreatitis, intestinal foreign object, or a bleeding abdominal tumor. I know it is hard to watch your dog be uncomfortable, but until there is a diagnosis I would be very cautious about using the Rimadyl. Hello and welcome to Petco Pet Education Center, formerly Petcoach. Coughing and bringing up clear liquid could be coughing up stuff instead of vomiting. This is important to try to differentiate. Coughing up liquid is more consistent with heart disease. The x-rays are difficult to read as the image transfer through is of poor quality and to really assess the lungs you need good quality images, however the heart does look a bit enlarged and the lungs do look a bit 'white'. I cannot assess the liver on these radiographs as you cannot see them. An enlarged heart, white lungs (fluid in the lungs), and an enlarged liver is consistent with both right and left sided heart disease. I recommend you return either to your vet, or another vet, and get a specialist ultrasound of the heart (an 'echo') to determine if heart medications are required. Your vet can also listen to the heart to see if a murmur is present in the heart, or 'crackles' are heard on the lungs. Heart disease is very serious and needs medications to manage. Without these Lily unfortunately will essentially drown in her own lungs, if heart disease is the problem. I suggest a recheck as soon as possible to get this figured out. Poor baby. With heart disease, their heart will sometime get larger and push up on their trachea and that will make them cough. You unfortunately cannot fix this. I would recommend to have your vet do some chest X-rays to see if this could be the cause of her coughing. Your vet will also be able to look at her lungs to see if she has any fluid in her lungs. If so, then she may need to be put on a diuretic to help with her coughing. I hope this helps!

The accepted dose of Rimadyl is about 1 mg per pound every 12 hours, so I would not give more than 50 mg in a 12 hour period. Rimadyl comes in 25, 75, and 100 mg tablet sizes and I don’t know which one you have. If she has already had an appropriate dose and she is still uncomfortable then she should probably be evaluated by a veterinarian as soon as possible. Best of luck and I hope she improves soon. It sounds as though Dixie is going through a lot at the moment! Yes, Dixie should absolutely continue taking the enalapril and furosemide, unless otherwise directed by the prescribing vet. The hydrocodone/homatropine medication is a cough suppressant, but may make her a bit groggy. The heart valvular insufficiency can progress to congestive heart failure, which can cause a cough to develop as a result of the fluid accumulation in and around the lungs. If her cough continues or worsens, Dixie may need to go back to her vet for another exam and chest x-rays, and may need additional medications to manage her heart disease. I certainly agree with your veterinarian that the symptoms you are mentioning do sound consistent with heart failure. If your veterinarian has not yet taken an x-ray of her chest, that would be the next thing I would recommend doing. It will confirm if there is fluid in her lungs. There are a number of medications that can be used for heart failure. A diuretic is only one of them and diuretic doses do have to be adjusted to each patient. It sounds like Ace might need medication at this point, but it can be really difficult sometimes to determine whether a cough is related to heart disease or respiratory problems like collapsing trachea or chronic bronchitis which can also be very common in senior small dogs. I would contact your vet and let them know that Ace is now coughing so much that he can't rest. They might recommend an echocardiogram to completely rule out heart disease and heart failure as a cause, or they might recommend trying some other medications for cough. If Ace has not had chest x-rays recently they would probably recommend x-rays as well.
