Should I worry about my old dog's enlarged nipples and testicular lump?

That looks like a growth, not a nipple. In a dog Rocco's age, there is always a chance any growth can be malignant and if it is, it does need to be removed surgically. Any suspicious lumps should be examined by the vet. The vet can perform a test called a fine needle aspirate on it. To do this, the vet sticks a small needle into the lump to get a sample of its cells, and examines the cells on a slide under a microscope. It's usually painless to the animal, and relatively quick to perform. Once the cause of the lump is diagnosed, the vet can discuss treatment options if needed. I would give his vet a call to make him an appointment to be seen in the next couple of days. Best of luck.

This may be benign or malignant but further testing would need to be performed on it (such as a needle aspirate or biopsy performed by your veterinarian). It has the appearance of a benign tumor but I cannot say for sure. In regards to the weight loss and not being able to hold his weight, there may be a number of causes for this such as cancer, systemic disease / infection, etc. I would recommend having a veterinarian examine George so that the most appropriate diagnostic and treatment recommendations can be made. I recommend having these seen by a vet as soon as possible to rule out cancer. Your vet can either remove both masses and send them to a lab for a biopsy, or take a small needle to collect cells and use that to make a diagnosis.
