Recently,my adult german shepherd dog have her black skin and a likely bald fur. I don't know why her hair is always falling and she always scartch it. She's getting weaker by now.

Changes in fur or skin color can often happen from chronic inflammation, which can happen with fleas, mites, food allergies, yeast infection, mites. It can also happen with endocrine disorders such as hypothyroidism or Cushing's Disease. Since Sabina is an older girl, I would recommend that she get checked out by your vet. If she isn't already on a monthly flea prevention, I would get that started. You can also try a medicated shampoo (see link below). If your vet does not know what could be causing her hair loss and itching after examination, they may recommend bloodwork to look for endocrine or other disorders.
