My dog has bad teeth & heart murmur. Swollen lymph nodes: what to do?

Her gums are likely inflamed because she has tartar underneath her gumline. She needs a dental cleaning to scale off all that tartar, then this will resolve the inflammation. I would suggest getting Chia an echocardiogram to see if she can still have surgery despite her heart condition. Many heart disease pets can go under general anesthesia. I would consider getting another vet for a second opinion here and see if you can get Chia's echocardiogram scheduled asap. I hope that your can get Chia's echo and dental scheduled soon. I would suggest using a water additive from your local pet store until then to help flush out her gums and keep the bacteria load down. Best wishes and take care!

Lymph nodes can become enlarged because they are draining an infection or inflammatory part of the body, because the lymphatic flow is being disrupted, or because of cancer in the nodes. I would discuss additional diagnostics with your vet, which may vary depending on which lymph nodes are involved. Aspiration of the affected lymph nodes is easy and can often be done without sedation. The samples are sent to a pathologist for review. X-rays of the chest and or abdomen may be indicated. Ultrasound of the abdomen may be needed. Dental cleaning and removal of diseased teeth if the lymph nodes are near the head.

Hi, Sorry to hear Dolly has this heart issue. Anesthesia is always risky, even for perfectly healthy pets. It puts a lot of responsibility for the heart to maintain adequate blood pressure and anesthesia is generally more risky with any heart issues than normal anesthesia. I would recommend that if the teeth just need a general cleaning, to not undergo the procedure and just maintain good dental health by brushing with doggie tooth paste and giving dental chews. If the teeth have a major issue such as a fracture or extraction that is necessary to address, it becomes balancing the risks of leaving a tooth inside that should come out, versus the risks of anesthesia. To fully assess the heart you can get an echocardiogram performed by a cardiologist and consult with them about anesthetic risks. Yes, that is a common presentation with a molar tooth infection. The tooth will need to be extracted and they will put Tootsie in antibiotics for the infection. Blood work is usually done before anesthesia and they will scale and polish the teeth and check the rest to make sure they are healthy. Once the mouth is healed then you can start brushing her teeth or using another dental product. They may also put Tootsie on pain medication. Heart murmurs can occur because of structural changes to the valves or chambers of the heart, which are relatively common in smaller dogs breeds. They can also occur because of anemia (low red blood cell number), infections in the heart, and unfortunately even tumors in heart. You are right in thinking that dogs with heart murmurs are at a higher risk of anesthetic complications, so your veterinarian will need to adjust their anesthetic protocol for any surgical or dental procedures. The only way to clear the infection would be surgical extraction of the tooth under anesthesia. I understand your hesitation to do so given the risk of his underlying disease. In this case, he will likely need frequent if not chronic antibiotic therapy. I don't like to use antibiotics unless absolutely necessary, but his infection will continue to recur and be a chronic source of pain for him. I hope this information helps!

The recommendation would be to remove the tooth but with the concurrent heart disease it makes the decision difficult to put him under anesthesia. I will say, I have placed numerous pets with heart failure under anesthesia that did just fine. This just required an adjustment of the medications and careful attention to the heart monitor to catch any changes early. I would not suggest pulling the tooth at home. I would recommend continuing antibiotics every other month to prevent infection in the mouth. I might also consider speaking with a dental specialist about removing the tooth. Need Dental Specialists deal with many high-risk cases. I would also make sure that you are addressing the discomfort with some kind of anti-inflammatory. If Chico likes playing with toys, I would provide Chico with hard treats or toys to chew on. These Carter objects may help dislodge the already loose tooth.
