Dog has holes on infected skin. Is it a bite or self-inflicted?

i am sorry to hear about your dog! It is very difficult to evaluate the skin by a photograph but it appears there is a defect/wound in the skin here? I would be concerned that this was an abscess that ruptures, a wound that isn't healing and is infected, or we could have something like a cuterebra (a fly larvae that lives in a hole in the skin- look closely and see if you see a tiny creature moving inside the hole!). I would recommend a visit to your vet- the area may need to be flushed/cleaned and have antibiotics prescribed. In the meantime, I would put an e-collar on your dog or a t-shirt if it would be covered by this to prevent your dog from licking/aggravating the skin. You can also clean the area with a mild soap or Hibiclens (available at the drug store) and apply an antibiotic ointment like polysporin to the area until your vet can see him.

That looks more like an infection either from a bite wound, a puncture wound, a foreign body stuck under the skin or perhaps a very deep fungal infection. I recommend to have Baloo see his veterinarian today. The doctor may be able to tell what the cause is by palpation and perhaps aspiration of some fluid. Then appropriate treatment will be given. It looks like the skin in this area has become infected. It could have started as a simple wound, like you said, and then progressively became infected. To help the area heal, keep the wound clean and dry. Wash it 2x a day with a gentle antibacterial soap. You can apply some triple antibiotic ointment to it as well. You will need to keep Boomer from licking the area as best as you can. The best way to do this is by placing an E-collar on him. If this does not help the area heal in 3-5 days or if the area around the wound is swollen at all, I would recommend taking Boomer to the vet to have it evaluated.
