My dog's skin fungus won't clear up. What should I try next?

There are a lot of things that could be causing this appearance to the skin, and it may not be easy or cheap to figure it out. I would recommend that you keep working with the vet. Baseline blood work, thyroid hormone, testing for Cushing's disease, skin biopsies, food trials, flea preventatives, etc may be needed.

You will first need to determine the type of infection. A recheck with your veterinarian would be recommended. Samples can be submitted for fungal culture as well as for bacterial culture and sensitivity. If it is a fungal infection, your vet can dispense an antifungal. Some of these infections can take weeks t months to resolve. If it is a bacterial infection, the sensitivity panel should determine the most effective antibiotic to use in treatment. Allergies should also be ruled out as an underlying cause. They can be frustrating to diagnose since they have multiple causes (food, environmental, parasites). Discuss allergy testing, food elimination trials, hydrolyzed diets and antihistamine use with your veterinarian. Legally, we are prohibited from recommending a medication in the absence of a physical exam. Any prescription medications will need to come from your vet.

Poor Buster! I'm sorry to hear that he's having so many problems. At this point, since he is on his third round of antibiotics without improvement, it sounds like he definitely needs some additional testing to help diagnose the problem. I would recommend a bacterial culture and sensitivity of the pus or discharge, as well as a punch biopsy of the skin to check for autoimmune diseases like lupus or pemphigus foliaceus. Depending on what else has been tried, other options might include a trial course of steroids or other immunosuppressive medication like Atopica or Apoquel, or a diet trial with a prescription hypoallergenic food. You can request referral to a veterinary dermatologist or get a second opinion from another vet locally if you feel that this is needed.

Hello. I am so sorry for your concerns with Brandi. I wish I could just diagnose her skin problems from just a picture, but this is quite impossible. If this has been a chronic issue, my recommendations would be to either get a referral to a veterinary dermatologist who specializes in unusual skin problems, or have your veterinarian biopsy one of these lesions. It is not unusual to have to go this route when you have a frustrating case that just does not want to resolve using conventional therapy. Fungal infections that are not responding to topical treatment often require a course of oral medications. Skin is the largest organ in the body, so it takes the longest to respond to infection treatments. Many skin infections require a full 6-8 weeks of treatment. Skin infections can also be complicated by allergies, underlying endocrine disease, or associated immune mediated or dermatological disease. Oh my. Poor guy. This infection is severe. I would have cultures taken of this skin immediately if the broad spectrum antibiotics that your vet gave aren't working. I would also consider getting biopsies of this thing. The area may need surgical debridement as well, because there is some diseased and necrotic (dead) tissue there that will prevent this from healing well if it isn't removed. I am so sorry your pet has such an aggressive skin condition. This certainly will need further medical management from your vet for sure. This is probably due to a severe allergy, although it could also be fleas or mites. First of all, in order to rule out skin parasites, you will need to treat Rufus with a high quality flea treatment (e.g. advantage), then get him to the vet to perform a skin scrape - this might reveal an infection or a mite infestation. If all of those came back negative, the next step is to treat the allergy symptomatically and try discovering the cause of the allergy. Some medications can be given by the vet in order to repair the skin lesions (steroids and antibiotics). simultaneously you should start him on a prescription hypoallergenic diet for at least 2 months. Hopefully you will see some results after all this, if not you and your vet should consider putting him on a long term allergy treatment (Atopica or Apoquel).
