Why is my dog scratching, red, and losing hair? What can I do?

It is possible that the underlying cause may be allergic skin disease and long term management may be required with medication, specialist shampoos for yeast and bacteria which are common in itchy/damaged skin and specialist diets and supplements such as viacutan. Allergies are possible to airborne pollen even in urban areas as well as foods and dust mites etc and cannot be ruled out in any location

Allergies and mange would be at the top of my list. A vet can easily check for intestinal parasites (mange) and then figure out a treatment plan for allergies if you don't have any parasites. There are many many different way to treat allergies, depending on how much you want to spend on medications/testing.

Skin problems can have a variety of causes, sometimes more than one. Allergies, parasites, bacterial or fungal infections, even contact with some surfaces or substances will cause itching, rash and/or hair loss. It is important to have the problem checked by your vet to determine if there is a medical cause for your pet’s skin issues and treat accordingly. In pets of all ages, fleas, food allergies and exposure to chemical irritants such as cleaners and soaps can be a cause. Any one of these may not be enough to trigger the breakouts, depending on how sensitive your pet is, but a combination can be enough to start the itch-scratch cycle. Finding out the cause and eliminating it is the best course of action. With flea allergies, if your pet is sensitive enough, a single bite can cause them to break out scratch enough to tear their skin. Check for fleas with a flea comb. Look for fleas and/or tiny black granules, like coarse black pepper. This is flea feces, consisting of digested, dried blood. You may find tiny white particles, like salt, which are the flea eggs. Applying a good topical monthly flea treatment and aggressively treating your house and yard will help break the flea life cycle. If you use plastic bowls, this is a possible cause for hair loss, though this tends to be on the chin, where their skin touches the bowl while they eat. If you suspect this to be the culprit, try changing the bowls to glass, metal or ceramic. Food allergies are often caused by sensitivity to a protein in the food. Hill's Science Diet offers some non-prescription options for sensitive skin as well as prescription hypoallergenic foods for more severe cases. Royal Canin carries limited protein diets that may also offer some relief. Your vet can recommend a specific diet that will help. If there is no relief or not enough, consider getting your pet checked by a veterinary dermatologist and having allergy testing done.

Itchy ears are most commonly a sign of allergies. Your dog may have food allergies, environmental allergies, or parasitic/bacterial skin infection. If he hasn't had a skin scraping to rule out parasites, or skin cytology/culture to look for bacterial or yeast infection, these things should be done. Some dogs actually have skin tumors or autoimmune skin disease that is causing the itching, and those dogs need to have fine needle aspirates or skin biopsies taken for diagnosis. Also, you should try a veterinary prescription allergy diet like Hills z/d for 8-10 weeks fed exclusively to treat potential food allergies. It's really important to feed ONLY the prescription diet during this time. Also, I have had great success with the oral pill Apoquel for some allergies as well, and this is a medication that is much safer to use long-term than steroids like prednisone. If this course of diagnosis and/or treatment is not possible with your current veterinarian, pursue a second opinion somewhere else. If all of these options are explored and your dog is still itchy, it's time to visit a veterinary dermatologist specialist.

It looks as if your dog has a skin infection, most commonly caused by an allergy, a skin mite (Demodex) or fleas. 

The allergy could be a due to a protein in your dog’s food or an environmental allergy like to dust, pollens, grasses, etc. 

The allergy causes your dog to itch and chew at the skin which breaks down the skin barrier. This then leads to infection. Skin infections are typically characterized by large areas of hair loss, redness, crusting, flaking, and pustules. A thorough skin examination by your veterinarian should be able to determine the cause of the itch so it can be treated appropriately. A skin scraping and skin cytology should be performed to rule out mites and to determine the type of infection present (bacterial vs fungal). Your dog will most likely need antibiotics/antifungals longterm to help clear up the infection and possibly an allergy/anti-itch medication like Apoquel, Atopica, or Cytopoint to give some relief while the infection clears up. A medicated shampoo will be beneficial as well. I would also recommend he be on a good flea control routinely too. Once the infection has resolved a long term allergy medication (like Apoquel or Cytopoint) can be given on a regular basis to prevent any future problems. I hope this helps.
