My dog's scrape isn't healing. He keeps licking it. Can I use antibiotics?

If your vet prescribed 28 pills after a surgery then you should give all the pills until gone unless otherwise directed by your vet. If the dog is scratching and licking at the incision then you need to put a cone collar on him and a socks on his back feet to prevent scratching. The cone needs to be long enough to go past the end of his nose or he will still be able to reach the incision. It looks like Scooby has ripped up part of the scab. I would gently wash the area with warm water and antiseptic soap, and put the Ecollar back on him. Keep them Ecollar on him until the area fully heals and there is no scab. The most important thing to do is stop his licking by using a cone. Then clean the wound with a warm damp cloth and apply a thin film of antibacterial ointment on it. I would recommend a plastic cone to go around Kali's head to prevent her from licking / chewing at the sore. She will likely need an ointment more effective than neosporin. I would recommend buying a plastic cone at the pet store and using an ointment such as Zymox topica ointment. If the sore worsens I would then schedule an examination with your veterinarian so that the most appropriate treatment recommendations can be made. It looks like he has a bacterial skin infection so he should be examined by your vet for treatment. Your vet can prescribe an antibiotic, anti-histamine, and/or a corticosteroid to help treat the condition. He could have allergies so he may need to stay on an anti-histamine long term. You could also bathe him with a medicated dog shampoo, like Epi-Soothe, and see if that helps. You could also add Free Form Snip Tips to his diet to help keep his skin and coat healthy.

This presentation could be due to a trauma or localized spot of skin infection (likely to be caused by bacteria). A check up, appropriate disinfection and possibly antibiotics (oral, topical or in a shampoo formulation) are needed to solve the problem. Disinfection may be started at home with very diluted hibiscrub or betadine or salted water baths. Buster collar may be useful as well to avoid any licking.
