Why isn't my dog's oozing leg infection healing with ointment?

Pet's info: Dog | German Shepherd | Male | unneutered | 3 years and 7 months old

Beneath my dog rear leg there is some infection going on and blood with fluid in small amount is oozing vet gave ointment and I am applying miconazole spray and ointment but not much improvement kindly see the attach photo and tell me what is it and its cause

I am sorry to hear about your dog. If this is not improving with topical medication he may need to be switched to oral/systemic medications as the problem may be deeper under the skin than topical meds can penetrate. I would let your vet know he is not improving. Occasionally we may need to do testing such as cultures or biopsy in some cases that don’t improve with traditional treatments.
