My dog has low fever. Vet gave antibiotics, but why no tests?

Poor guy. Not every pet with a fever has an infection, so if the antibiotic isn’t working it’s either because there is no infection, the infection is internal, or the antibiotic is not the right kind of antibiotic for the infection. Your pet needs additional testing to sort this out and make sure we are on the right track with treatment. I would start with baseline bloodwork and a urinalysis. Best to take your pet back in to your vet or to another vet for a second opinion here so that we can get a proper diagnosis and tailored treatment based on your pets specific problem. I'm sorry but Riko's symptoms are very non-specific so it is very difficult for me to know may be going on with him just based on your description. Blood tests and x-rays do not always give a diagnosis. That is why there are so many different testing modalities available. So, I would say you should start with chemistry and CBC blood testing and x-rays of his abdomen to check for any obstruction.

There are many causes for fever other than bacterial infection, viruses, inflammatory diseases and autoimmune conditions can also contribute. Blood cultures are not an efficient test unless there is an overwhelming blood borne bacterial infection such as septicaemia and fecal cultures can become overwhelmed with normal/ non-disease causing bugs but if not responding, a fecal culture and microscopic exam may identify underlying causes but should be at least 5days after the last antibiotic. Specialist diets, fiber and probiotic supplement such as fortiflora could also be considered

Antibiotics are usually indicated if there is obvious signs of a skin infection, GI infection (if pet has loose stool and parasites are not the cause), upper airway infection, or a UTI. I am not sure of any vets that I know personally who would just give antibiotics if there isn't just cause. Bloodwork is not always needed to justify the use of antibiotics, unless we are suspicious of bacteremia (bacteria in the bloodstream). Besides bloodwork, there are some specialized tests available, which are usually cultures, however they are often not cost effective for most clients to afford, ranging anywhere from $100-$300. In those cases, we treat based on symptoms and the history of the pet, as we narrow down the possibilities of an infection vs something else. Also not to mention that some of these specialized tests (i.e. cultures) have an incubation period of anywhere from 7-14 days before results are available. If we don't treat for what we highly suspect, the pet will end up just getting sicker while awaiting results, if the owner can afford the test to begin with. The same thing is done in human medicine. If the doctor highly suspects an infection based on the physical exam and vitals, then antibiotics will be prescribed. On another note, we have to make sure we do our part and ask all the right questions about the pets history and rule out other causes before issuing antibiotics, so that we used them judiciously and not increase the risk of creating superbugs in the environment. My next step would be to send off blood for a test for plague (Yersinia pestis) and Tularemia, especially if your dog has access to areas where there are rodents, squirrels and rabbits. These would not be included in regular blood work. You may also want tod discuss a disease called malignant hyperthermia with your veterinarian. Other common causes of "fever of unknown origin" would be tumors, however your vet apparently ruled those out with blood tests, ultrasound, and xray and possibly blood parasites such as Ehrlichia spp, Babesia spp and similar. A heartworm test may also be advisable.

General tests to consider include bloodwork (CBC and chemistry panel), urinalysis, thoracic radiographs and ideally abdominal ultrasound. This would be a good place to start to see if the cause can be found. An infection is possible though other things like inflammatory diseases or cancer can also cause a fever. This sounds like a pretty complex situation, and I'd agree that an infection of some sort seems likely with a high WBC. I'd be worried about something viral, perhaps even meningitis, especially if it seems that she has neck pain, but that's basically a guess on my part. She should be tested for tick-borne diseases (Rocky Mountain Spotted Fever, ehrlichia, babesia, Lyme) if that hasn't been done. I'm not sure how far you are from a specialty hospital, but it sounds like she's still not out of the woods, and in the absence of a definitive diagnosis, I too would be worried. I would recommend taking Sky to a specialty hospital, where they might be more capable of diagnosing the problem and getting her the appropriate treatment.
