My dog keeps licking her paw. What will a vet do for infection?

Hello! I'm sorry to see that your dog is having some trouble with her foot. It appears to moist and irritated between the toes, and there is also some swelling around the nail beds of two of her nails. I am very concerned that this may be a skin infection, and possibly also a nail bed infection (which can sometimes be tricky to treat because of how deep the nail goes into the toe). I do not recommend continuing the Zymox spray without the advice of a veterinarian, as it contains hydrocortisone (a steroid). Steroids will decrease inflammation, but they also suppress the immune system. I strongly suggest that you bring your dog to visit the vet as he or she may require antibiotics and/or antifungals to treat any potential infection she has. In the meantime, you can try soaking her foot in cool, mildly soapy water, followed by thorough drying. If your dog is licking the foot, I recommend placing an Elizabethan collar (E-collar) on his or her neck to prevent further licking, which can exacerbate inflammation and contamination with bacteria and yeast. At the vet's office, they will perform a physical exam, and most likely they will take a sample of skin cells and discharge from between those toes and around the nail bed for examination under a microscope. This will help them determine what treatment will work best. They may also give your dog some pain relievers or anti-inflammatories so that he or she is more comfortable. Usually a veterinarian would perform a cytology to distinguish between a bacterial and fungal skin infection - they would take a sample on a microscope slide and examine for organisms. It's also possible that Bini has a resistant bacterial infection, or she might have another underlying disease process like an autoimmune disorder contributing to her symptoms. In your picture her foot does not appear classic for a fungal infection. You may want to follow up with your vet to discuss further testing and treatment.

Poor girl. I do feel that culturing this area would be best, that way we know what antibiotic is needed to rid the bacteria present for sure. Especially after being on the same antibiotic for 28 days already. Re-infection of an area is also likely if the pet continues to lick at it. Therefore, if the infection is completely gone, you have to then treat the pets itch. If we are licking our feet due to allergies, then the underlying allergy should be addressed by keeping her on allergy medication. Discuss with your vet about the options of either Cytopoint injections or Apoquel to help manage her allergies. Also soaking her paws in Chlorhexidine solution 3% diluted 1:10 with water daily can help soothe her paws and remove any allergens she picks up on her feet from outside. Chlorhexidine wipes can also be purchase and used to help with this. In the meantime, I would opt to go with a stronger antibiotic prescribed by your vet, perhaps Simplicef should be tried, until you can decide on further testing/treatment. I would also highly recommend having Bini tested for thyroid disease, which can cause chronic skin infections, lethargy and weight gain in dogs. This presentation could be due to a trauma or localized spot of skin infection (likely to be caused by bacteria). A check up, appropriate disinfection and possibly antibiotics (oral, topical or in a shampoo formulation) are needed to solve the problem. Disinfection may be started at home with very diluted hibiscrub or betadine or salted water baths. Buster collar may be useful as well to avoid any licking.

From your picture I'm concerned about infection, and often oral antibiotics are recommended for skin infections. While you're waiting to see your veterinarian the main thing you should do is try to prevent Lucy from licking, which will be easier said than done! Even though the hard plastic e-collars look sad, most of the time they are the best way to prevent licking. You could also try putting a dog bootie on her feet if you think she would leave it on.

The most likely cause would be a secondary bacterial or yeast infection secondary to allergies give your description. A bad odor is usually associated with overgrowth of bacteria or yeast and requires veterinary exam in order to get the appropriate medications to treat the overgrowth and also to get medication to get the inflammation under control. Until you get an appointment with your vet you should put an e-collar on her so she doesn't further traumatize the feet.
