My dog has jaundice, fever, lethargy, negative tests. What's wrong?

Maxi certainly sounds quite sick, but I am glad to hear that she is eating and drinking normally. As I'm sure your vet discussed, tick borne diseases are one of the more common causes of a fever of unknown origin (FUO). However, FUOs can also be caused by other infectious diseases (toxoplasmosis, Bartonella, Brucella, mycoplasma, fungal diseases, etc.) as well as immune-mediated or cancerous causes. To answer your question, yes, you will need to do some more diagnostics to get to the bottom of this issue. Different causes of the FUO will require different treatments. Maxi's vet will be able to guide you as to their next tier of diagnostic testing based upon their clinical suspicions. I hope that all goes well!

My next step would be to send off blood for a test for plague (Yersinia pestis) and Tularemia, especially if your dog has access to areas where there are rodents, squirrels and rabbits. These would not be included in regular blood work. You may also want tod discuss a disease called malignant hyperthermia with your veterinarian. Other common causes of "fever of unknown origin" would be tumors, however your vet apparently ruled those out with blood tests, ultrasound, and xray and possibly blood parasites such as Ehrlichia spp, Babesia spp and similar. A heartworm test may also be advisable.

Based on this bloodwork I can see that she is anemic, has elevated inflammatory cells, has low chloride and sodium and perhaps a slightly elevated creatinine (which can signal kidney issues though I cannot read it well there). These are not indicative of any disease in particular. This can be due to an infection for sure though. Other possibilities include an inflammatory condition, among other possibilities. I highly recommend you contact the veterinarian that has been treating her to discuss this bloodwork and proceed with therapy accordingly as she has not improved completely. Oh goodness. I am sorry Junior is so sick. There can be multiple problems causing the signs you are seeing. Intestinal obstruction, pyometra (uterine infection), liver or kidney disease just to name a few. I would highly encourage you to have her evaluated again by your veterinarian. At this point I would say that x-rays and blood work are very important to make a diagnosis. If she is this sick it could be life threatening without proper diagnosis and treatment. It definitely sounds like Lyra is sick, but her symptoms aren’t consistent with any specific disease process. I would recommend taking her back to the vet. Rechecking lab values will be helpful to see if there are any changes, and IV fluids would be helpful to fight inflammation and dehydration. She may need additional diagnostics like x-rays or an ultrasound to see what could be going on internally.

Hello and welcome to Petco Pet Education Center, formerly Petcoach. Hepatitis (infection and inflammation of liver) is a very serious condition and is very hard to treat. If there is an internal medicine specialist in the area I would recommend having Nala seen by them. Otherwise I would be talking with your vet about testing for infectious diseases. However, it looks like they have treated her with broad spectrum antibiotics as well as IV fluids. I would recommend continuing treatment. She may need a feeding tube placed to try to keep her protein up during this illness. Unfortunately, with how severe this is, she may or may not pull through it, but the best chance of her making it through is to be aggressive with her treatment. I would suggest continuing to monitor her liver and white cell count, and to have imaging done and possible samples of her liver to try to determine the cause of the infection. Otherwise I would be talking to your vet about the parasites and infections present in your area and get covered with certain antibiotics. The white cells have increased however not that much, and the liver enzymes are actually improving if I can read the dates right from your picture. This is very hopeful. I would continue with antibiotics (IV if you cannot get them in orally), as well as IV fluids and I would think about syringe feeding or placing a feeding tube. I am so sorry your pet is so ill. She needs to have some bloodwork done at this point and possible further testing (ie urinalysis +/ x-rays) to try and find the underlying cause for her symptoms. Also another antibiotic may be needed if her infection is not responding to the current one chosen. If bloodwork has already been done then please request a private Consultation with either myself or another expert here for a more in depth conversation as to what the bloodwork results indicate for a second opinion. She may even need to be hospitalized and give IV medications until she is well enough to go home and receive oral medications readily .
