My dog has inoperable heart disease. How can I best care for her?

Heart disease affecting the lungs occurs when the heart is unable to pump the blood forward well, and the pressure back-up in the lungs causes fluid to seep out into the lungs, and the lungs fill up like a sponge filling up with water. The medications for heart disease include a diuretic which will essentially 'ring out' that sponge. It pulls fluid out of the body, and therefore out of the lungs. For this reason Rocky will drink and pee a lot on the medication. The other medications work by making the heart pump better (Pimobendan is the usual drug of choice for this) as well as decreasing the blood pressure so the heart can pump in a forward direction more easily (ACE inhibitor usually). Depending on the level of heart disease your vet will choose 1, or all 3, of these medications. Some dogs respond very well to medications and can live for a while, with changes in the medications every once and a while. Some dogs unfortunately don't respond well to the medication. It all depends on how severe the heart is at this point, as well as how well Rocky will respond to medications. You can get an echo (ultrasound of the heart) to determine exactly how bad the heart is at this point, or just see how he responds to the medications. His best chance is to be diligent about his medications, don't let them run out as he will need to be on them for the rest of his life, and do your follow up appointments as your vet is recommending. I'm so sorry Duke is having heart and lung problems! I am very concerned that he has not eaten for several days and has no energy. He may potentially have something else going on besides his previously diagnosed heart and lung issues. I would recommend you take him back in to your vet as soon as possible to be evaluated. Often with heart disease, dogs need a combination of medication, including furosemide and pimobendan (the cardisure), and potentially a blood pressure medication. If he is not eating he may need more aggressive treatment and potentially injectable medication. He also may need another x-ray to see how his lungs look. He may need blood work to make sure his kidneys are okay as well as check his electrolytes. Again, I'm so sorry Duke doesn't feel well. Heart disease can be difficult to manage and is progressive, so over time medications sometimes have to be adjusted and/or dosages changed. I hope he starts feeling better soon. You could ask them if they work with a cardiologist. If she hasn’t yet had an echocardiogram (performed by a cardiologist) that will probably give you the best information. Don’t hestitate to respectfully and politely ask the vet in charge of her case for clarification if you don’t understand what’s going on with her. Sometimes we forget to discuss things in an understandable manner! I am sure your vet would be happy to clarify as best as possible. I’m sorry I don’t have more specific answers, but with complicated cases like hers I really can’t give you a whole lot without knowing the entire story. (Which can’t be done in 500 characters.

There are many medications which can be used to support a failing heart and should be used alongside diuretics in order to help support the function of the heart, reduce fluid build up in the lungs and abdomen and control blood pressure if required. Unfortunately, no cure is available and medications should be adjusted as Zoe's condition changes; many dogs can live for months-years with a heart condition depending on the severity of other health complications which can also occur as they age. Without a full history of all her complaints and recent tests it is impossible to assess her medium-long term prognosis

Hello and welcome to Petco Pet Education Center, formerly Petcoach. Coughing and bringing up clear liquid could be coughing up stuff instead of vomiting. This is important to try to differentiate. Coughing up liquid is more consistent with heart disease. The x-rays are difficult to read as the image transfer through is of poor quality and to really assess the lungs you need good quality images, however the heart does look a bit enlarged and the lungs do look a bit 'white'. I cannot assess the liver on these radiographs as you cannot see them. An enlarged heart, white lungs (fluid in the lungs), and an enlarged liver is consistent with both right and left sided heart disease. I recommend you return either to your vet, or another vet, and get a specialist ultrasound of the heart (an 'echo') to determine if heart medications are required. Your vet can also listen to the heart to see if a murmur is present in the heart, or 'crackles' are heard on the lungs. Heart disease is very serious and needs medications to manage. Without these Lily unfortunately will essentially drown in her own lungs, if heart disease is the problem. I suggest a recheck as soon as possible to get this figured out. I'm so sorry to hear that Lily has been so sick. Based on the bloodwork that you've provided, it looks as though in addition to elevated liver values, she's also quite anemic (low red blood cell count). It is important to determine why Lily is experiencing this degree of liver disease. Given her relatively young age, issues like infectious diseases are most likely, but cancer or other immune-mediated issues are also possible. I would recommend asking your vet about running some testing for different infectious diseases, like Mycoplasma, Bartonella or Cytauxzoon. In the meantime, also ask your vet if Lily is at the point where she needs a blood transfusion to manage her anemia. This decision is based on vital parameters, exam findings, as well as her hematocrit (HCT). You may also want to ask your vet if Lily looks as though she needs a diuretic, like furosemide, to remove excess fluid from in and around her lungs. Lily's medical case sounds quite complicated, so I'd recommend seeking out a second opinion on her care, if available. You can also ask your vet to contact veterinary internal medicine specialists if they need more specific advice for her diagnostics and treatments. I hope that she's on the mend soon!

Poor Luna sounds like she isn't doing well at the moment. I would take her into the vet first thing in the morning. Her quality of life does sound poor at the moment, and maybe putting her to sleep is the best option. But discuss it with your vet first, they may have additional ideas as to how to make her quality of life better. I would be prepared, however, that it may be time to let her go. I hope this helps.
