My dog has a heart murmur, meds aren't working. What are my options?

Heart murmurs do not always require therapy but when they do, these are most of the time treated with medications. Hi there! A murmur just means an abnormal sound was detected when the vet listened to the heart with his/her stethoscope - it can be caused by many different things. The most common heart problems in dogs that could cause a heart murmur would include valvular insufficiency (a "leaky" mitral valve, usually), aortic or pulmonic stenosis (narrowing of the opening to the aorta or pulmonary artery), or dilated cardiomyopathy, in which the heart becomes stretched out and doesn't contract strongly anymore. Most of the heart problems we see in veterinary medicine are not correctable by surgery, and open-heart surgery of the type that is done on humans is not done in veterinary patients - so unfortunately this isn't an option. To determine what is causing the murmur and see if any treatment is needed, I would recommend talking to your veterinarian about chest x-rays and an ultrasound of the heart, called an echocardiogram. These tests are needed to diagnose the specific problem that is causing the murmur, and will guide treatment decisions from there depending on what is found.

I am so sorry to hear about what is happening with your pet. If your dogs diagnosis is congestive heart failure, then the usual pills that the vet will place a dog on our furosemide and enalapril. And if the correct diagnosis is congestive heart failure, then these pills do a very good job of controlling the clinical symptoms associated with this disease. However, there are many different heart diseases that a schnauzer can get. Some of these are mitral valve disease, heartworm disease, etc. The only true way to diagnose the disease is to have a set of x-rays as well as an echocardiogram which is a heart ultrasound. The good news and what you have going in your favor is that the most common disease in a miniature Snells or is congestive heart failure. And if this is the case, the pills that I assume you are on will most likely be able to keep her around for quite a while. I cannot give a prognosis as to how long, but your odds are pretty good that we are going to survive for a period of months to years. Thank you for reaching out to us. I hope I have helped.

Poor guy. The coughing is mainly due to fluid bulid up in the lungs. If the coughing has not improved then ask you vet if adjustments can be made to his current medications, in particulra the Libeo (Furosemide), which is designed to pull fluid off the lungs, reducing the cough. He is on all the medications currently recommended for heart failure dogs, so the best thing to do is increase his medications and perhaps get him on a cardiac prescription diet (i.e. Hill's Science Diet H/D). Also understand that heart medication is not curing the disease, it is merely managing it to buy your pet more time. Unfortuantely the medication may reach a point where it is no longer effective when the disease has progressed too far. I do hope that we have not reached that point yet, and perhaps can still make adjustments in Kenzo's medications to help manage his heart disease for a while longer. This would need to be discussed with the treating vet but such a significant heart murmur will require ongoing management and lifelong medication which is not inexpensive. It is likely to have an impact on both overall life expectancy and exercise intolerance, the underlying heart disease cannot be cured and symptomatic treatment aims to reduce the severity of the symptoms and slow down progression. If finance (or a very active lifestyle) is a concern then you should think very carefully before adopting such a dog.

Dogs can have heart murmurs for a number of reasons (physiologic murmurs, murmurs due to anemia or low red blood cell count), but the most common cause is due to a structural issue with the heart. This is most often associated with the heart valves. A murmur occurs because the valves aren't functioning properly, causing the blood flow to be more turbulent and produce an audible sound (when ausculted with a stethoscope). An ultrasound of the heart (called an echocardiogram) allows a veterinarian to look at the structure of the heart and determine the degree of dysfunction in any of the valves. Some dogs do not require medication if the structural issues are mild, but if they are severe, your veterinarian may recommend certain medications to help Tracker's heart function the best it can and therefore keep him alive and well as long as possible. Heart disease is tough. This is true for humans or animals. Pets can quickly go from ok to not ok in minutes. Most forms of heart disease are progressive which means they get worse over time. Signs of worsening heart disease can be increased heart rate and respiratory rate, collapse, exercise intolerance, or coughing. There are many medications that can help improve quality of life.
