If he continues to behave in this manner, its best to get him seen b your vet for a full physical exam to try and localize the painful area and get her on pet friendly pain medication to help her get more comfortable. If Alfie is laying his front legs down with his rear legs extended in a 'play bow' position this could be a sign of abdominal pain. He could also be having pain in his rear legs that makes him unable to lay down easily. At his age I would recommend having him examined by a veterinarian sooner rather than later to look for a source of pain and to prescribe medication to help.

Some dogs can groan or make noises like this when they are changing position, and it may not be significant. If he seems to have having difficulty breathing, if he is using his belly more when breathing out, or if he seems to be painful you may want to take him to the vet. Otherwise I think you could continue to monitor him and mention the change to his owner when you're done petsitting.

It's very possible she is experiencing some pain. You didn't mention how old Kourtney is, but arthritis is very common in older dogs. Other issues such as inflammation, slipped discs, injury, etc can also cause pain and stiffness. If the symptoms continue, or she seems in distress, scheduling a visit with your vet is best. They can check for the underlying cause, and get her started on a combination of medications such as glucosamine for joint issues, or pain and inflammatory meds to help her feel better.
