Why is my dog gagging and reverse sneezing after heartworm treatment?

Updated on September 23rd, 2025

Pet's info: Dog | Boxer | Male | 55 lbs

My dog went to the vet for heart worm treatment while I was in class yesterday morning from 9am until about noon. On the way home he started hacking and gagging and threw up once. He can breathe normal some times but it is now 3am the next day and he is still sounding like he's choking or doing a reverse sneeze noise and then gagging. Is something stuck in his throat? He has been given the vaccine for kennel cough months ago.

What you are describing sounds like a reverse sneeze. Typically, we are concerned with patients developing respiratory distress associated with heartworm treatment secondary to worm die off and blood clot formation. It does not sound like a that is the case here. Nonetheless, contact your veterinarian and let them know. He may need to be evaluated if the sneezing and gagging continue.
