Dog with enlarged heart & trachea collapse still coughing: what to do?

Hello and thank you for your question. I hate it that the medications are not providing cough relief for your dog. In my experience, if these two medications are not stopping the cost of a tracheal collapse patient, then surgery should be considered. there are so many great surgical methods and treatments now for collapsing trachea. One of them being a mesh wire coil that can be placed in the trachea to keep it dilated without the possibility of collapse. These are usually performed by surgical specialists and your general veterinarian may have to refer you to one of them. I agree that Scotch's trachea is severely narrowed on this x-ray, which is likely associated with his coughing. Without seeing another x-ray view (preferably with Scotch on his back), I can't comment about his heart size or shape. There appears to be additional soft tissue opacity around his heart on this x-ray view, which could certainly be partially due to heart enlargement, but could also be associated with pneumonia or a heart base tumor. Scotch has significant dilation of his esophagus as well. It is important to determine the underlying cause of Scotch's cough and chest abnormalities in order to determine the best course of action. A different x-ray view is the next diagnostic step, potentially followed by an echocardiogram (ultrasound of the heart). Based on those results, Scotch may need a course of antibiotics to treat pneumonia, different medications if heart failure is suspected or chemotherapy if a heart base tumor is present. I imagine that your vet suspects congestive heart failure given that he is giving Enalapril and Lasix. I don't understand the need for Gardinal (phenobarbital) in this situation unless Scotch has epilepsy. I hope that all goes well for Scotch!

There is no way of knowing how long this may have been developing but it is quite likely the condition developed in line with the worsening cough and breathless signs. Tracheal disease is common in this breed also and it is not unusual for both conditions to develop in the same dog, weakness and coughing at rest is generally considered indicative of a worsening heart condition which requires ongoing medical management to reduce symptoms and delay progression

Poor baby. With heart disease, their heart will sometime get larger and push up on their trachea and that will make them cough. You unfortunately cannot fix this. I would recommend to have your vet do some chest X-rays to see if this could be the cause of her coughing. Your vet will also be able to look at her lungs to see if she has any fluid in her lungs. If so, then she may need to be put on a diuretic to help with her coughing. I hope this helps!

Poor guy. If the Temaril alone is not helping him I would highly recommend telling your vet so that they can also prescribe additional medications. Tracheal collapse is often treated with several medications in more severe cases. Cough tabs or Torbutrol tabs are given to help reduce the cough. Also a bronchodilator is helpful in some pets to open up the airway just before the thoracic inlet. For now you can mix a small amount of honey mixed with water and offer that to Ferris until you an get him seen again by your vet. Honey has anti-inflammatory proerties that may be somewhat helpful. Also make sure not to place a collar on him, harness only, and try your best to keep him in a quite room with limited stimulus. I hope that Ferris feels better soon! Best wishes and take care.

If it has been going on for years it is probably a collapsing trachea, in which case there is not much that can be done about it. There are medications that can be used to minimize the cough but it will not "fix" the problem. There is a surgery that can be done to place a stent in the trachea which is curative. A collapsing trachea is not a huge problem other than uncomfortable for them to cough. It should not cause any major issues. You can ask your vet to do a trial of medication to see if it helps and if it does, you can keep her on them long term. Based on the radiograph you attached, the vertebral heart score (VHS) is on the larger measurement and overall the heart does have increased contact with the sternum. There does not appear to be evidence of heart failure, but I am limited with one x-ray view and no ability to adjust the contrast. I don't know what medication your veterinarian started Bono on but in this case, I would recommend starting omega 3 fish oil, Pimobendan (prescription medication that helps delay on the onset of heart failure and helps the heart pump more efficiently). I would also look into heart supplements such as Standard Process Whole Body Support and Cardiac support (if available in your country).
