My dog's disc pain is severe, and gabapentin isn't working. Help!

My cavilier king Charles has a disc problem. She's been on gabapentin for two weeks for the pain. She's also on prednicare and they work. The gabapentin seems to have stopped working. This is her fourth day of pain and she's now looking worse than I've ever seen her. She tried tramadol but started fitting on and of in quick succession for twenty minutes and she kept stopping breathing. I have no faith in my vets because the attack was critical and due to her prescription and yet she said it woul

I am sorry to hear that Oucy is in so much pain. Disk disease is a very painful condition and is typically treated on an emergency basis. Tramadol is usually not very effective for spinal cord pain and often animals need intravenous pain medications to get some pain relief. While gabapentin can be helpful for nerve pain it is an adjunctive pain medication and needs to be used in combination with something else.

The best thing you can do for Oucy is to get her to an emergency vet clinic for surgery and supportive care. Even if surgery is not an option, medical pain management would help her feel more comfortable. I would not recommend waiting to take her in. In the meanwhile be sure to follow your veterinarians exact instructions for the prescribed medications and do not give her any human pain medications as these can be toxic to animals.

    Hello, sorry to hear about Oucy. If she seems to be doing worse then perhaps she may either need more aggressive pain medications that have to be given in the veins or have surgery. I would recommend taking her to a specialty center whom may have a neurologist whom could examine Oucy.

    Disk disease is very painful and may not completely respond to medications alone. It truly does depend on the severity of the injury to the spinal cord from the disk disease.

    In the interim, keep Oucy quiet and on soft beddings. Do not allow her to jump, walk, go up/down stairs, for then next 3-4 weeks.

    I think it is time for Oucy to see a specialist in your area. If theres a neurologist available, that would be best. Hope this helps and best of luck.

Unfortunately eye pain can be difficult to treat - oral medications sometimes don’t seem to make much difference. There is some evidence that tramadol is not metabolized well in dogs so I think gabapentin might be a reasonable alternative. Ultimately healing the ulcer will be most important - if it does not seem to be healing well you may need to see a specialist to have a burr procedure or keratotomy in order for it to heal. Topical pain medication might be another option but can sometimes slow the healing process.

Is it safe to give my dog Tramadol after Gabapentin for pain?
Tramadol and gabapentin are safe to give together. Tramadol comes in 50 mg tablets. The upper limit of the dosing range for a 78 pound dog is 150 mg. However, I rarely start with the high dose. You should take her to your veterinarian for a re-assessment. THis may include changing to another NSAID, or adding other pain medication such as Gabapentin, or even opioids. You may also start with some physiotherapy such as swimming in warm water, to keep her mobile. Unfortunately more and more evidence points to Tramadol not being very effective in dogs for taking pain. I suggest you make an appointment with your vet soon.

My dog's IVDD pain is worsening. Should her current meds be changed?
Yes, it is possible that the meds are not strong enough, personally those medications would not be my first choice for IVDD as this is usually a very painful condition and these medications are not strong enough to treat this type of pain. i would recommend contacting you vet in order to switch the Codein with Tramadol and add either Metacam or Rymadil to the treatment as well.

Can my older dog safely take Gabapentin, Tramadol, Metacam daily?
If this was prescribed by your vet , then this is the amount of medicine he needs. Gabapentin may make him a bit drowsy in the beginning, but that should subside after about two weeks. Gabapentin is against nerve pain. Tramadol is an opioid which takes some pain as well and Metacam is a non-steroidal antiinflammatory which takes away inflammation and therefore also pain. These medicines are safe, however watch your dog's poo in the first two weeks. If it is black or tarry then stop the Metacam and inform your vet.

There are a few more medications that can be given such as tramadol and gabapentin, both are prescription only, you can get them from your normal vet. the most important factor for good recovery is cage rest for at least two weeks, in most cases neither MRI nor surgery are needed.

Tramadol has been found to have very little effectiveness in pain control in dogs. So less and less vets are prescribing it. Of course humans are abusing it, and it is a controlled drug. But the main reason is that it just is not that effective.
