My dog has Cushing's. Why is she restless, not eating, and falling?

Unfortunately Cushing´s syndrome is caused either by an adrenal tumor, but more often by a brain tumor. Nystagmus (eyes moving from side to side) could be a sign of the brain tumor expanding. Also that she is not steady on her feet could be part of that. On the other hand there is also a chance that this has nothing to do with the Cushing;s disease bu that your dog is suffering from geriatric vestibular disease, which is harmless and should go away by itself within a few weeks. With vestibular syndrome dogs often get nauseous and will not eat, so she may need some medication to take away the nausea.

As you know, Cushing's disease causes excessive cortisol (steroid) production, which can lead to an increased appetite. With the cortisol levels under control, Lucy's appetite may have regulated, but 13 pounds is a pretty significant weight loss. Normally, you should be able to easily feel (but not see) a dog's ribs and see a waistline when looking down on them from above. We can easily become accustomed to seeing overweight dogs as "normal" and can perceive a healthy weight as being too thin. (Without examining Lucy, I can't comment as to if she is truly underweight, or if her weight is appropriate.) Since her appetite has become picky and her Cushing's disease seems to be well controlled, there is also a chance that she has developed some primary intestinal issues (such as inflammatory bowel disease) chronic pancreatitis, or intestinal parasites. At this point, I recommend taking Lucy to her vet for another exam to assess her overall body condition and consider additional diagnostic testing (CBC, chemistry and abdominal ultrasound) to try to get to the bottom of this issue. The random yelping isn't really typical for Cushing's Disease. These symptoms are usually weight gain or pot-bellied appearance, thinning hair, increased hunger, increased thirst, increased urination, and occasionally increased respiratory rate/panting and restlessness. I would recommend that you have Miley examined by a vet. The symptoms you describe sound like pain, which could be coming from multiple places, such as joints, the back, inside the abdomen, or chest, or could be caused by neurologic or other musculoskeletal issues. If the vet is also concerned about Cushing's Disease, then bloodwork can be obtained and tests scheduled to look into this possibility.

My biggest concern is that your vet is talking about hyperadrenocorticism also known as Cushing's disease. This is what I think of when you discuss those symptoms, liver enzyme elevations and adrenal issues. Cushings is where they produce too much cortisol in their system. You can do a test to rule this in or out. You can have your vet do a low dose dexamethasone test OR a ACTH stimulation test. If it comes up positive then you can treat with the disease with a daily medicine called trilostane to help the symptoms. But you cannot cure cushings. Some common symptoms of Cushings's are panting, increased thirst/urination, but these are also common symptoms of many diseases. Bloodwork and endocrine testing can help to ferret that out. Neurologic symptoms can sometimes be present with Cushing's if there is a larger tumor in the pituitary gland but seizures can also be caused by other things as well so an MRI is typically required. CT can sometimes be used if a pituitary mass is strongly suspected. Itchy feet is typically caused by allergies which may also be associated with secondary infection. Next steps are to see your vet for an exam, bloodwork, and endocrine testing. Abdominal ultrasound and blood pressure measurement are also typical next steps. Cushing’s syndrome (if that is indeed what she has), is rarely an emergency. Many dogs have had the disorder for months (or years) prior to diagnosis. However, diabetes can have very similar signs and that is more of a concern. If she has lost weight, or if she is not eating then I would recommend an emergency veterinarian to make sure she is not diabetic and starting to Decompensate. I also notice that she is not spayed. Pyometra is a uterine infection that usually starts with increased water intake, But they usually progress to not feeling well soon. Some of these dogs also have vaginal discharge and typically occurs 6 weeks or so after last heat cycle. If she is eating well and seem fine otherwise then waiting until Monday seems appropriate to me. If possible try to take a urine sample with you to your veterinarian appointment.
