My dog is sick for weeks with cough, fever, weight loss, no diagnosis.

My next step would be to send off blood for a test for plague (Yersinia pestis) and Tularemia, especially if your dog has access to areas where there are rodents, squirrels and rabbits. These would not be included in regular blood work. You may also want tod discuss a disease called malignant hyperthermia with your veterinarian. Other common causes of "fever of unknown origin" would be tumors, however your vet apparently ruled those out with blood tests, ultrasound, and xray and possibly blood parasites such as Ehrlichia spp, Babesia spp and similar. A heartworm test may also be advisable.

Poor Dagul! I am sorry that he doesn't feel well. Looking at the blood work his white blood count and kidney values are all elevated. The kidney values could be elevated simply due to dehydration but I cannot say for sure since I cannot see him myself. The white blood count being so high makes me concerned about a severe upper respiratory infection or even pneumonia. These blood results look like they were done in February so they really aren't very accurate now. I would strongly recommend have him seen again by your vet if possible and recheck the blood work. Your vet will be able to do a thorough exam and let you know whether or not he has a good chance of making it. Your vet may even recommend x-rays to ensure there is no tumors in the lungs causing issues as well. I am really concerned about him. I would also make sure he is continuing to eat and drink to help keep his hydration levels up. I hope this helps and he starts to feel better soon!

I would recommend that Rex have a thorough checkup with a vet. Gaining weight and lethargy can be a symptom of hypothyroidism, which can be confirmed easily with a blood test. However, the shortness of breath has me a little concerned that there could be heart or lung disease also at play. Heart disease can cause fluid build-up in the lungs or abdomen, which can easily be confused for weight (fat) gain. After thorough examination, the vet may recommend bloodwork to check for internal medical illness, and x-rays or ultrasound to see what is going on inside the body. After your vet has a better idea of the cause of Rex's symptoms, then an appropriate treatment plan can be discussed.

Many medications would take longer than 5 hours to see a positive response, but it varies based on the presumptive diagnosis your vet has made and the particular medication. Poor guy. Not every pet with a fever has an infection, so if the antibiotic isn’t working it’s either because there is no infection, the infection is internal, or the antibiotic is not the right kind of antibiotic for the infection. Your pet needs additional testing to sort this out and make sure we are on the right track with treatment. I would start with baseline bloodwork and a urinalysis. It is very hard to say what the cause of Rajah's symptoms may be without examining Rajah and running current blood work on him first. The high globulin means there is an inflammatory process occurring in the body. The high amount of immature or new red blood cells means he is using up red blood cells faster than he can supply them. This may indicate a bleed somewhere internally or may also indicate a cancerous process occurring or a severe infection. Rajah's current symptoms are concerning and should be treated very soon with your veterinarian. Sooner rather than later would be better. I would recommend in addition to blood work, an x-ray, or abdominal ultrasound to look for a tumor or free fluid in the abdomen. I hope this helps and I wish you and Rajah the best of luck.

You might ask your veterinarian if the X-rays could be reviewed by a radiologist. While the resolution that the forum pictures allow makes them a bit difficult to interpret, I am concerned that the X-rays do not appear normal, and she may have a more serious underlying condition. It may be that she has some underlying heart disease, cancer, or an infection that requires a more broad spectrum of antibiotic. I agree the blood work you've attached is unremarkable. Was a CBC performed as well? Check back with your vet about a radiology review, and a recheck exam as well. A consult with a cardiologist may be recommended, depending on the radiologist's opinion.
