Why are my senior cat's crusty ears getting worse after antibiotics?

This looks like a possible bacterial or yeast infection now. This can happen secondary to a primary issue such as mites. If the antibiotics are helping, it's probably a middle ear infection indeed. a sample needs to be taken from the ear and sent for culture and sensitivity, then the best antibiotic can be chosen according to the result. the course of treatment must be longer next time, at least 4 weeks as these infections take a long time to resolve.

Ask your veterinarian if these have been ruled out at this point. If not, look into testing for these. Crustes around the eyes can indicate a viral infection. I would be worried as well. I suggest you have Penelope back to the vet for a follow-up. It looks like the ear flap is red and swollen. It is not likely ear mites because only one ear is affected. In over 25 years in the veterinary industry I have never seen only one ear affected with mites. Something else may be going on. Hopefully it is just a mild infection from the scratching but why she is scratching.....I don't know. Without examining her myself it is hard for me to say.

A lesion like that could be a bacterial infection or ringworm (which humans can get). Your best option is to have Mocha examined and tested for ringworm. Your kitty has either a bacterial infection above his eye or ear mites making him scratch his ears and this area raw. Look inside the ears, and if there is black nasty crusty discharge, he has ear mites. The infection above his eye needs to be seen by a vet for antibiotics and possibly even allergy treatment. If he has mites, he needs a Revolution or ivermectin treatment to kill them. Hope that helps!

This presentation could be due to a trauma or localized spot of skin infection (likely to be caused by bacteria but fungal infections are possible as well). A check up, appropriate disinfection and possibly antibiotics (oral, topical or in a shampoo formulation) are needed to solve the problem.
