My 4-month-old kitten is weak, thin, and falling. Does she need a vet?

we found a kitten like about 4 months old. She's so thin and whenever I let her eat, she only eats few. Whenever she walk, she walks slowly and when she jump from my lap to the floor, I thought she will land but she instead fell and could barely stand but she's still trying her best. really need to do something to help her. Is there anything I can do without taking her to a vet? Please help me..

Unfortunately no, there is nothing you can do without taking her to a vet. She sounds quite ill, and she truly has to have veterinary care; in fact, I would consider her current symptoms a medical emergency. She sounds very weak and she's likely starving, and she needs veterinary care now if you want to help her. Please take her into a vet right away.
