Indoor cat scratching a lot, no fleas. What could be wrong?

Since your veterinarian was able to actually take a look at Sophia, I would believe their diagnosis. It could be that a lack of consistent response to therapy could be that the allergic component is not yet being addressed. Her staying in the litter box is likely due to discomfort which is why re-evaluation at this point is indicated. Your veterinarian could consider referral to a veterinary dermatologist for further recommendations. It really comes down to figuring out what is causing her allergies so that they can be treated or the specific allergen avoided. There are many things that can cause allergies though in most cases they can be broken down into 3 big categories: flea, food and environmental allergies. Allergies are usually worked up in a stepwise fashion starting with flea allergies as this is the easiest and cheapest to take care of. If not on a flea preventative, then it would be a good idea to get started on one. If already on one then it would be a good idea to change to something else (there are many new products out in the market). If not improved in 1-3 months then it is time to move on and check for food allergies by switching to a novel diet (new protein and new carbohydrate). If that does not seem to be the issue then you are left out having to check for environmental allergies which require blood and/or skin tests. Allergies can be very frustrating to deal with at first but so rewarding at the end when we are able to treat them effectively!

It sounds like you have done the right things to treat fleas if Willow had them, and I would be thinking that there is another cause of her itching. Other causes can include mites, ringworm, or allergies. I would have her seen by a vet again for this itching. The vet can do blood tests, skin scrapings, and use a Wood's lamp to diagnose allergies, mites, and ringworm. Once the cause of the itching is found, the vet can prescribe the appropriate treatment.

Hello! Sounds like you are doing a great job trying to sort this out! What I usually do in these cases, is treat for mites and fleas regardless of whether we are seeing them (sometimes pets are allergic to a certain parasite, so they itch like crazy with just a few fleas, which could be hard to find on exam). Not sure which products you have in Australia, but your vet would know what is safe for your cat! (here, I would use something like Revolution!). Beyond that, I'm not sure! Some sort of allergy really fits well, but with the new symptoms of head shaking and sneezing, a virus could be brewing as well. Would you be able to call your vet today and ask about a topical flea/mite treatment? (Just make sure it is something your vet recommends-they know your cat best!)

First of all, in order to rule out skin parasites, you will need to treat with a high quality flea treatment, then ask the vet to perform a skin scrape - this might reveal an infection or a mite infestation. If all of those came back negative, the next step is to treat the allergy symptomatically and try to discover the cause of the allergy. Three main type of allergies are usually affecting pets: food allergies, flea and parasite related ones and environmental type (allergens coming from trees, pollens, dust etc etc). Some medications can be given by the vet in order to repair any possible skin lesions (like steroids and antibiotics). I am sorry to hear about your cat! I would encourage getting her on a flea preventative even if you aren't seeing fleas because they can be very difficult to spot and some cats are very sensitive to flea bites and can have just one bite can make them quite itchy. In terms of over the counter products you can try, I would consider starting an omega fatty acid supplement such as Welactin, which can decrease inflammation and itching, and consider using a topical product such as Duoxo Calm mousse or spray on the area if bathing is difficult. I am sorry to hear about Jewel. Fleas are definitely a possibility in any pet who is itching, even if they are supervised when outdoors, a flea can easily jump onto a pet without our notice. They can also be very difficult to find on a pet if there are just one or two fleas. In addition, some pets are very sensitive to flea bites and just one or two bites can make a pet very itchy. Other possible causes could include allergies to things in the environment such as grasses/pollens/dust. If your kitty has red lesions/spots/scabs/bald patches on the skin I would recommend an examination by your vet. If you don't see any skin lesions you could try using a flea preventative (your vet carries several types such as Comfortis, Bravecto, Revolution or you can purchase a product such as Advantage or Frontline at the pet store- be sure to purchase a product specifically for cats).
