My Persian cat has a swollen belly with fluid. What's the cause?

Thank you for using Petco Pet Education Center, formerly Petcoach! From the X-rays, I can see that Mara's abdomen appears to be filled with fluid. It is obscuring his liver and intestines from being seen and there may even be a small amount in his lungs. Unfortunately it is impossible to tell what the fluid is from the x-ray alone. His blood work shows some abnormalities with his electrolytes, which may be if he is not eating or from fluid in the abdomen. Mara also has a high white blood cell count, these are the cells that are responsible for fighting off infections and increase if there is one present. Your vet can run some additional diagnostics, and perform a tap and remove the fluid as well as a blood test to determine if Mara is infected with FIP, a mutated form of a common virus that cats are exposed to. Hi there. The spleen is a lymphatic organ, somewhat similar to lymphnodes. It can enlarge with many different types of problems such as feline leukemia virus, FIV, FIP, and even cancers like lymphoma. If blood work has not been done yet then I would recommend blood work and an ultrasound of the abdomen. I would encourage you to discuss doing an ultrasound guided needle aspirate of the spleen with your veterinarian. This may help determine what the underlying problem is. I'm so sorry to hear that you and Sam are going through this scary situation. With normal blood work and the normal ultrasound, I would be concerned about the disease called FIP (Feline Infectious Peritonitis) which is an uncommon response to a common virus in which the abdomen fills up with fluid and cats don't tend to recover from it. I would also continue to be concerned about cancer even though you're not seeing any signs in the fluid or on the ultrasound. A tumor has to be at least a centimeter in diameter before it can be seen an ultrasound, so it's not unusual for something to just be too small. Unfortunately, without a diagnosis, it's incredibly difficult to treat. And even with a diagnosis, at his age, it's mostly bad things that it could be. At this point, your vet is probably focusing on palliative care to keep Sam comfortable by removing the fluid, keeping him well hydrated, making sure he is comfortable. I would recommend talking to them about his prognosis and your goals for him.

Poor Lucky! There are several causes for a swollen abdomen like parasites and fluid in the abdomen. I would have your vet take a look at him as soon as you can to see what the cause is. If there is fluid in the abdomen, it could be a serious condition that needs to be treated. It could also be FIP which isn't treatable but he needs to be seen to be diagnosed. Hopefully, since he is still eating, he just has some intestinal parasites which your vet can easily treat. I hope this helps and he feels better really soon.

I understand you are concerned about FIP. The reality is, if you don't have money for a veterinary visit (and in many cases even if you did), FIP is typically fatal and untreatable. The best thing you can do is separate your sick cat from your healthy one and see how things transpire over the course of the next few days. Typically, cats with FIP, regardless of the form, will be inappetant and not wanting to eat anything at all. The worst form of it will cause massive fluid buiid up in the chest / abdomen and result in difficulty breathing. The other form typically produce clinical signs that are both ophthalmic and/or neurologic, plus several other non specific clinical signs (a couple of which include what you are descrbing). Overall, FIP would not be near the top of my list. I would definitely make sure all three cats have been tested for leukemia and AIDS however in order to rule both of these life-threatening conditions. While not as rapidly fatal as FIP, both conditions can carry poor long term prognosis. It sounds like the one cat with a fever and swollen belly might benefit from pain management and a recheck for the presence of worms. While dietary indiscretion could be an issue, it is not likely to cause a fever unless an infectious organism was present in the food. Hope this is more helpful than my previous response.

In my opinion going to the ER would be best, although you may not be able to have a full ultrasound until Monday morning. Some potential causes of fluid in the abdomen include an infection or abscess in the abdomen, diseases like FIP, or cancer. If there were an infection it would be best to start treatment sooner rather than later. An ER vet could take a sample of fluid to examine for signs of white blood cells or bacteria and might be able to perform a limited ultrasound. It's also possible that a specialist could be called in to perform a full ultrasound if needed over the weekend.

Fluids in the abdomen can be caused by a lot of things and it usually makes the xray non diagnostical, at this age FIP can be one of the causes but it could also be a severe gastrointestinal infection or a liver problem. The next stage will be a specialist ultrasound, fluids might need to be drained first.
