White spots on cat's spay incision 4 weeks later. Is it normal?

She may be licking excessively which could be contributing to the inflammation. Cats have rough tongues which can traumatize the skin leading to infection. Place an e-collar (cone) on her to prevent licking and have her rechecked by your veterinarian. She may need antibiotics and/or anti-inflammatory medications to resolve the issue.

I would guess that these skin ulcerations are from licking and not related to the surgery three months ago. I’d recommend having your vet take a look in person. It’s possible that Beezus has allergies or another skin condition causing a secondary skin infection.

All in all, I think that Tokyo's incision looks perfectly normal for being about 10 days post-op. I can see that she's likely been licking the center part a bit more than she should have been since the tissue is still slightly red. However, it's still healing nicely. Since this area is a bit red, I would try to put an e-collar (cone) or baby onesie on Tokyo for the next few days to prevent her from licking at it. The white areas that you're seeing look like normal fibrin-rich tissues, which are normal in the healing process and will fall off shortly. I hope that she continues to heal well!

However, this should resolve/improve within a few days. If at her 10-14 day post-operative, her skin is still not looking healed, your veterinarian may chose to perform additional diagnostics at the site. Alternatively, if your kitten is licking at the area, she may be causing some irritation to the skin there. Please make sure she is not licking the area by placing an e-collar on her if necessary. Monitor the are for any swelling, redness, discharge and contact your veterinarian if you note any as this could indicate a local infection or reaction to the suture material. She could have a infection or allergic dermatitis. You could bathe her with a medicated cat shampoo and see if that helps. Otherwise, have your vet take a look at her to determine the cause and treat the problem. She may need an antibiotic and/or anti-inflammatory to treat.
