Why does my indoor cat have a single skin scab that won't heal?

Hello, my name is Dr. Elizabeth and I am happy to help. I hate to hear this lesion on Patch is not healing. I think the main thing to address is the licking. The first thing you need to do is stop the licking (since this can make it worse). Patches should wear an elizabethan collar at all times. Remember that the collar must come out past her nose in order to be effective. Next, wear gloves to handle the lesion and be sure to wash your hands afterwards. Get pet wipes that contain Chlorhexidine. Cleanse the area twice a day. This will help to control the number of topical pathogens as well as remove oil and dead skin cells. Lastly, after the area is completely dry, apply a topical antibiotic twice a day. Just a dab will do and rub it in well. Neosporin is fine to use. Again, wear gloves or apply with a q-tip. Patches needs to be indoors at all times. Cats can't wear elizabethan collars outside as they are a choking hazard if they were to get hung up. She can only wear the collar if she is in a safe environment. If the spot is not much better in 1 week, if it gets worse at any point or if the lesion is not completely healed within 2 weeks, Patches needs to be examined by the veterinarian. Since the lesions are getting worse, I recommend o have Theo examined. It's likely these lesions are infected although something like an allergic reaction, ringworm (which humans can get) or skin mites are also possible. Fleas and allergies to fleas can set-up an environment for infection. It's possible that this is complicating the recovery process. The skin will often start to heal once the fleas are gone, but it's worth treating for infection by giving a bath with a medicated cat shampoo (can get this at the pet store). I recommend discontinuing the use of the other products (except the flea control) for now, because overuse and misuse of certain products can actually delay healing or make the problem worse. For instance, I worry about the use of the coconut oil, because fungal and bacterial infections will thrive in the coconut oil. It's best to just give a bath with the medicated shampoo once every 2-3 days, until the skin starts to look normal. It's expected that it will take a month or more until the cats start to look normal again, so stick with it and they should get through this fine. Treating for fleas is not just restricted to the host, because fleas can reproduce and nest in the environment and continually cause an issue. Skin lesions like you are describing can have a variety of causes, including skin parasites (fleas or mites), an infection (bacterial or fungal), allergies, or another underlying medical issue. I would recommend he be evaluated by his regular veterinarian to determine the underlying cause for his skin lesions, so he could start any treatment (antibiotics, anti-itch medications, parasiticide) that he may need. In the meantime, I would clean the area will diluted betadine if possible and then keep it dry, and trim his nails back as much as possible to keep him from scratching at it. Good luck!

Ouch! Those are some major hot spots in that photo. I would recommend having Lili seen by your vet so the area can be clipped and cleaned and any medications such as pain or antibiotics can be given to help it heal a bit faster. I'd also keep Lili from scratching or pawing at it as this can introduce bacteria and more inflammation which can slow healing. If only Lili is affected and you're not noticing other issues like parasites, etc, it is possible there is an allergy underlying. She could be rubbing up against something causing the itchiness, or could have an ingredient allergy to something in her food (or a treat if the spots correspond with intermittent feeding of a treat or wet food). Other issues such as an underlying illness like an immune or metabolic issue can also cause scabbing and skin issues that affect only one cat in the household. A vet visit is a good option at this point both to treat the current scabs and to see if a food trial, allergy meds, or additional testing such as bloodwork to rule out any underlying health issues may be beneficial in finding the cause.

Scabs like this could be caused by allergies (food allergies, or an allergy to something in the environment), or a bacterial or fungal skin infection. If you're positive that he doesn't have any fleas, then I would recommend having him examined by a vet when you are able to. It sounds like he will probably need antibiotics and a short-term steroid or other anti-inflammatory medication to treat the problem, and the vet can also talk to you about options to prevent this issue from recurring in the future - such as a diet chance to a prescription hypoallergenic diet, or a daily allergy medication if needed.

She could have a foreign body or infecion in that area that won't properly heal. I would have your vet look at it to ensure she doesn't need some antibiotics or even a culture of the area to ensure it isn't a resistant infection. Finally, if it won't heal, she may need that area surgically removed as I have had a similar case where we surgically removed it and sent the tissue to the lab and there a piece of grass was embedded in that tissue.
