My cat has severe liver and kidney infection. Is there hope?

The most important part of treatment when an animal has kidney failure is to give adequate fluids. It would be best to keep Leo at the vet to stay on IV fluids until his creatinine comes down - if the kidneys are not receiving adequate fluids, you are not likely to get him to improve. If you must have him home, then ask your vet if they would be comfortable teaching you how to give subcutaneous fluids at home (injecting an appropriate amount of saline under the skin) to help him stay hydrated and keep flushing his kidneys. If he is eating, then you can add extra water to his food to encourage more fluid intake. However, if he's not eating, he REALLY needs to continue getting fluids, either intravenously at the vet or subcutaneously at home. If neither of these is an option, and he's not eating, then unfortunately euthanasia should be considered. I hope this helps, and again I'm sorry Leo isn't doing well.

Unfortunately I would need to know a little more about her particular case in order to give more specific advice on treatment and prognosis. Acute kidney injury can be caused by toxins or infections, and with aggressive IV fluid therapy and hospitalization sometimes the kidneys can recover full function. Chronic kidney disease happens more slowly, and while diet changes and monitoring for secondary problems like high blood pressure can help slow the progression of disease there is no cure. ALT and cholesterol can increase for a number of different reasons - gastrointestinal infections, pancreatic disease, and liver and gallbladder infections would be a few. It seems like her kidney disease is likely more significant at this point. Part of this decision should be based upon how Squeakers is doing clinically and how bad or not bad his values are. In general, if kidney disease is caught early on, it responds very well to treatment. Most of the time, treatment is simply a diet change to a kidney friendly, prescription diet. Pet’s where the disease is more advanced, may need to take a pill to decrease protein loss through the kidneys and they may need subcutaneous fluids on a daily or weekly basis. For cats with more advanced disease, they may need hospitalization and IV fluids to see if they will recover. Again it all depends on the blood values and how the pet is doing clinically. Most cats with kidney disease, if caught in time, will live an average of 2 to 3 years of quality life after diagnosis. This is variable though. Hi, Oscar needs to be hospitalized and treated with IV fluids, antibiotics, and gastrointestinal protectants. Additionally, he should be tested for infectious diseases such as leptospirosis and ultrasounded to determine if he has any problems in his liver or kidneys.

I think IV fluids and supportive care are a reasonable place to start. If your veterinarian is suspicious of acute kidney injury then Mimi could regain kidney function with aggressive supportive care, but some cats will worsen despite treatment. If your veterinarian is suspicious of chronic kidney disease then her kidney function may not improve but often cats can be managed with medication and diet changes with a good quality of life for months to years. The acute vs. chronic designation is usually based on the history and overall exam findings. I think an ultrasound and urinalysis are good next steps - you may also want to ask about culturing the urine (although she may currently be getting antibiotics which could change the results, I can’t quite read a couple of the injectable medications). A blood pressure might also be helpful, and checking phosphorous, calcium and electrolytes in a blood panel could help. The only lab finding that is a little odd is the increase in lymphocytes, although sometimes the machine will not differentiate between cell types well and this may not be significant. It looks like he is suffering from some liver and kidney values elevations and he also is anemic. (Low red blood cells). Since I do not not his current condition and how long this has been going on, it is difficult for me to give you a survival time. If this has happened quickly and could be a toxin then with aggressive therapy (hospitalization and IV fluids), then these values could go back to normal and stay that way as long as there wasn't any permanent damage. If this has been chronic and his urine is very dilute then this could be permanent damage to the kidneys which is a much poorer prognosis long term. I would still recommend hospitalization and IV fluids if possible. I would talk to your vet about whether he needs fluids, antibiotics, appetite stimulants, anti-nausea medicine etc. I hope this helps and your sweet baby gets well really soon!

Sorry to hear about your kitty. It sounds like he is in the advanced stages of kidney failure. Unfortunately kidney damage can not be reversed once it occurs - only managed. Once cats get to this stage of kidney disease, most treatments are focused on trying to make cats feel as good as possible for as long as possible. Sometimes treatments that are prescribed include SQ fluids (water given as an injection under the skin to increase hydration), anti-nausea medications, blood pressure medications, antacids, and other therapies to manage complications associated with kidney failure. It is hard to give a prognosis without knowing the specific details of your cat's bloodwork and history, but if he is not eating or drinking, he may be suffering and can quickly decline. If you are concerned about your kitty's quality of life, have a discussion with your veterinarian.
