My cat has daily seizures. Phenobarbital isn't working. What can I do?

Based on the history, a toxicity seems less likely and a primary disease of the brain more likely. If the seizures are not well controlled on phenobarbital it may be that she needs a higher dose. You should discuss phenobarbital levels with your veterinarian. If the levels are in therapeutic range, a second anticonvulsant such as levetiracitam should be considered. Furthermore, you may consider following up with a veterinary neurologist to discuss diagnostics including an MRI of the brain.

I'm sorry to hear that Maci is having this problem! Idiopathic epilepsy, which refers to seizures without any underlying cause, is rare in cats but can occur. Other possible causes would include a brain tumor or neurologic disease of some kind, as there would not be any evidence of these issues on normal bloodwork. If you are concerned and want to try and get a definitive diagnosis, you can request referral to a veterinary neurologist for further testing, which would likely include an MRI or CT scan of the brain as well as a CSF (cerebrospinal fluid) tap. If going to see a specialist is not an option for you, your veterinarian should still be able to prescribe medication to control Maci's seizures even without knowing what the underlying cause might be - I would definitely recommend this at this point, if she is having seizures every other day.

Hi and thanks for your request. The aim of the phenobarbital administration is to reduce effectively seizure frequency and severity. In my experience a frequency of once a month is fair and acceptable. Ideally Phenobarbital concentration should be checked 3 and 6 months after starting the treatment to monitor liver parameters and make sure the concentration of the drug in the blood is within therapeutic ranges. Dose could be even increased, but to be able to give you specific advices about it I would need to know Shadow's weight. Thank you for submitting your question regarding your dog. I recommend contacting his veterinarian. He will likely need to start another medication to control his seizures. Phenobarbital, potassium bromide and zonisamide are common choices. However, your veterinarian will know his medical history and which would be the best choice. You will need to take Freddy back to your veterinarian for a blood test checking phenobarbital levels and possibly an adjustment of his medication or change of medication. This can only be done with a hands-on exam. If you are able to, video one of these cluster seizures and take the video to your vet so he can see them.

It is common for dogs with epilepsy to eventually need more medications added to their anti-seizure medication protocols. There are several anti-seizure medications that can be considered including phenobarbital, potassium bromide, Keppra, zonisamide, among many others. I would encourage you to discuss with your veterinarian about other anti-seizure medications. Often times dogs with epilepsy respond favorably to other medications. Good luck and have a great day!
