Is routine cat anal gland expression a chronic issue or minor problem?

Pet's info: Cat | Domestic Shorthair | Female | spayed | 14 years old
Would routine anal glands cleaning (1 or 2 mo. visits) for a feline, be considered Tx for a big or little problems OR a chronic illness management? And under which category: Veterinarian Hsp or Wellness Clinic?

If Barri Bonds (great name!) needs his anal glands expressed due to scooting, infections or abscesses I would consider that a chronic problem. Cats should be able to express them on their own. I would consider it under wellness not hospitalization. If he only had them expressed once and we think he won't have to have them done often then it would be a "little" problem and still under wellness.
